Women Can Protect Their Health With Recommended Screenings

Medical professionals stress the importance of women getting the proper screenings to protect their health and discover problems early when they may be more treatable. Screenings are designed for otherwise healthy individuals and recommendations change as we age. 

An article in the publication US News & World Report, states, “Your doctor can help you tailor the recommendations based on your goals of care, personal and family health history, age and life expectancy.”  This post discusses 4 of the 14 screenings in the article.  

Breast cancer screening with a mammogram is recommended annually for women from age 40-50, every two years for those 50 to 74 and on a case-by-case basis after that.  Doctors say mammograms are key to the prevention as well as early detection of breast cancer, when it is most easily treated. Talk to your doctor about testing options if you are under the age of 40 and have a mother or sister who had breast cancer at a young age or have a high-risk genetic marker.

It is recommended women 50 years and older have a colonoscopy every ten years. As colon cancer is more common in African Americans and occurs at an earlier age, it is advised to start testing at age 45. Younger women/teens with a family history of adenomatous polyposis – a genetic condition that starts in someone as young as in the teen years may need colonoscopies at a much earlier age. Also, a woman with a history of Crohn’s disease or inflammatory bowel disease will likely be advised to have a colonoscopy at a younger age than 50. There are also other screening tests for colon cancer which may be right for you.   

Diabetes is another disease a woman 45 years and older should be screened for every three years. Exceptions for screening earlier are a family history of diabetes; obesity and high blood pressure readings or you take medications for HBP.

A Lipid profile assessment testing a woman’s cholesterol and triglyceride levels as well as the risk for heart disease should be done every five years starting at age 45, or younger for those at high risk. If lifestyle changes do not reduce your numbers, you may need medications. As with all screenings, discuss with your doctor how often you need to be screened and what steps to take to improve your health.

While your doctor will have plenty of questions to ask you during your next appointment, experts say there are questions you should be ready to ask. The following can help you stay healthy or get well quicker. 

For example during your next annual physical, ask if you need any medical screenings. Also should you be taking any vitamins or supplements? Dr. Jessica Edwards, founder and CEO of Zara Medical, a hybrid primary care practice, says the answer can be different for each person. She explains if you are a vegetarian you may need a vitamin B12 supplement or if you live in an area without much sun, you may need vitamin D, and so forth. 

If you have a less common condition, ask how many people the doctor has treated with your disease. If not many, inquire about seeing a specialist who is familiar with the condition. This is also a good time to ask about treatment options and side-effects of any medication you are being prescribed. Moreover, what is the next step if your symptoms do not improve or get worse?

If you are having tests done, ask when you can expect the results, and if they will be found on your online portal or will you get a phone call. Based on the results, will you need a follow-up appointment?

Regular Screenings Help Keep Older Adults Healthier

As we age doctors recommend seniors get some additional health screenings. However certain tests may no longer be required for very elderly people so be sure to talk to your doctor about what is appropriate based on your medical history and age. 

The American Academy of Ophthalmology recommends adults 65 years and older get a thorough eye exam every 1-2 years as problems increase with age. Ophthalmologists warn serious diseases such as glaucoma can silently steal our vision without noticeable symptoms. An exam also looks for signs of cataracts, age-related macular degeneration and diabetic retinopathy. 

Colorectal cancer screening is recommended for adults starting at age 50 and continuing until age 75 (if you’re older than 75, it may depend on your history). Tests include colonoscopies, fecal immunochemical tests (FIT), or flexible sigmoidoscopy.

Breast cancer screenings are recommended at age 50 to 74 every 1-2 years, or based on family history of breast cancer. For men, prostate cancer screenings recommendations vary. The American Cancer Society says, “The decision should be made after getting information about the possible benefits, risks, and uncertainties of prostate cancer screening.” 

Lung Cancer screening is recommended for people at high risk or a history of smoking. Skin cancer checks by a dermatologist are recommended for seniors at least once a year. A bone density test to screen for osteoporosis is recommended for women age 65 years and older, and for men age 70 and older. Repeat scans should generally be performed every 2 years.

As the prevalence of diabetes increases with age, diabetes screening is important. The Cleveland Clinic recommends screening for diabetes at age 45 and over, with follow-up tests every three years. Those with a BMI greater than 25 and additional risk factors may need more frequent testing.

Hearing screenings are recommended every 5 years until age 65, every 1-3 years after 65, and more frequent testing if there are risk factors or problems. Test results are used to monitor hearing changes over time and provide a picture of a person’s hearing health.  

Other routine screenings include cholesterol and blood pressure as well as annual cognitive and mental health assessments. Talking About End-of Life Issues With a Loved One

Medical professionals and counselors who deal with families facing end-of-life issues offer a few suggestions to the rest of us on “how to start a conversation” when we have a very sick loved one. They say the first thing to consider is having the conversation in one’s kitchen is a whole lot easier than having it in the hospital. It may also encourage your family to start the conversation if they learn that studies find families who have talked about it beforehand express less feelings of guilt after the person passes away. Moreover, they say they had an easier time grieving and were less likely to experience depression. 

The type of questions to ask to get a conversation started include “what does the person value most about their life?” Other topics of discussion could be to find out what the person understands about their illness; what matters most to them at the end of life; how they want to handle medical decisions; and what their fears and physical needs are. 

As a family member, you need to remember to practice active listening. It is important to leave your own views out of the discussion and try to understand what the person is feeling. Find out if they want to spend their last days at home; and who do they wish to be surrounded by as they face death. This is also a good time to encourage them to express their wishes regarding burial. 

Ask if the person has an advance directive, which is a legally binding document that gives instructions for healthcare if they are no longer able to make or communicate those decisions themselves. This can include their wishes around discontinuing treatment and medications; and saves the family from having to make difficult decisions.

Scammers Often Target Older Adults

The Federal Trade Commission reported in 2023, adults in the U.S. lost a record $10 billion to fraud. They want us to remember scammers are skilled and persuasive, and use tactics designed to catch us off guard. 

A common scam is to phone older adults and pretend to be from organizations like the IRS, Social Security Administration or Medicare. During the call, they ask for personal information, and if the person is reluctant to give it, they threaten to cut off benefits. Once they have your information, they use it to commit identity theft.

Be wary of calls from someone who claims you have won a lottery or prize and the only hitch is you need to send money or gift cards to cover taxes and processing fees. Also, don’t be taken in by a popular scam claiming your computer is compromised and you need to pay them to fix it. Especially targeted at seniors is a caller pretending to be a grandchild or relative (or their friend) pleading for money to be wired to get out of jail or other types of trouble.   

To protect yourself, change your passwords often and make sure they are complicated. Use two-step verifications when you have the option. Be careful of giving out any information over the phone, in emails or through texts. Avoid transferring money or purchasing gift cards over the phone. Experts say once you complete such a transaction, the money is typically unrecoverable. Finally, if you are targeted by a scam, notify your local police department or reach out to the National Elder Fraud Hotline.

Women and Bone Health

Endocrinologist Kendall Moseley, MD, medical director of the Johns Hopkins Metabolic Bone & Osteoporosis Center, says most women do not know anything about their bone health. A recent survey from the National Osteoporosis Foundation backs up that sentiment finding around 94 percent of women do not worry about their skeletons. However, they add they should, as one in five women under 30 already have osteopenia, which is a step above full osteoporosis. The good news is there are things you can do to protect your frame and avoid the things that are not good for bone health.  

Experts say our bones constantly break down and rebuild every 10 years resulting in a new skeleton. The problem lies in the fact that after the age of 30, our bodies start to lose more bone than they make. Women’s estrogen levels also begin to drop around 30, which is another protector of women’s bone health. 

An unbalanced diet or severely restricting calories in an effort to stay thin or lose weight can be another cause of low bone density; and too much caffeine, salt and alcohol all interfere with retaining calcium, our primary bone builder. Even some medications deplete calcium and include certain anti-depressants and contraceptives so be sure to ask your doctor about side effects of medications you take. It is important for women to include dairy products or foods like beans and nuts to increase their calcium intake. Protein from plants and lean meats are also good ways to get calcium.

As We Age, Protecting Our Health is More Important Than Ever

As we age, protecting our health is more important than ever. We know older adults tend to experience severe symptoms and complications from many communicable viruses and diseases, and can have a harder time bouncing back from them. Being vaccinated lowers the risk for serious illness, and in extreme cases death; and the side effects of vaccines are typically mild and temporary. While Medicare covers the cost of vaccines for the flu, COVID-19 and RSV, many seniors are not always sure if or when they need to get another shot. 

The Center for Disease Control and Prevention (CDC) recommends people 65 years or older get an annual high-dose flu vaccine if available as they suffer the most from the disease. There are two types. One high-dose flu vaccine contains an antigen called hemagglutinin and has 3 to 4 times as much flu virus antigen as a standard vaccine. The other has an ingredient called an adjuvant that helps the immune system respond better to the virus. If you are over 65 and suspect you have the flu, talk to your healthcare provider as soon as possible as they can prescribe a flu antiviral treatment. 

The current recommendation for the COVID-19 vaccine is for people 65 years or older to get 2 doses of the updated 2024-2025 vaccine six months apart. They say is you have upcoming travel or events in your life where you want to be very protected from the disease you could get your second dose somewhat earlier (minimum interval of two months). 

The respiratory syncytial virus (RSV) vaccine protects against the respiratory syncytial virus, and the CDC recommends all persons 75 years or older get the one time shot. If you are someone at an increased risk, they say people ages 60-74 years old can get the RSV vaccine. 

Other vaccines covered by Medicare Part D include vaccines for shingles, tetanus-diphtheria-whooping cough (TDAP) and others. Consult with your healthcare providers on what vaccines you should be getting to protect your health.

Many People Incorrectly Believe Dementia is a Normal Part of Aging

The World Alzheimer Report 2024 published by Alzheimer’s Disease International states eighty percent of the public think dementia is a normal part of aging. More troubling is that 65 percent of health care professionals believe the same. While the Alzheimer’s Association estimates 1 out of 9 Americans age 65 and older suffer from Alzheimer’s, remember that leaves a greater number of us not getting the disease.  

Howard Fillit, M.D., clinical professor of geriatrics, medicine and neuroscience at Mount Sinai School of Medicine in New York City, and chief science officer of the Alzheimer’s Drug Discovery Foundation, explains, “Certain abilities —processing speed, for example — slow down [around] the age of 20 or so, when processing speed peaks; to age 70 or 80, when processing speed can be down as much as 50 to 70 percent.” On the flip side, seniors typically have superior vocabulary and decision-making skills. 

The National Institutes of Health funded a study to discover if individuals close to someone with dementia looked at it differently. They concluded that “people with a family member with dementia were less likely to believe that dementia is preventable and less confident about the effectiveness of taking preventive actions.” These unfavorable perceptions discouraged them from engaging in healthy behaviors linked to a decrease in a person’s risk for Alzheimer’s. Prior studies also found family members of a person with dementia were more reluctant to get early screening, which is too bad as today’s treatments are best started in the early stages of the disease. 

While it is true a direct relative of someone with Alzheimer’s is more likely to develop the disease than those without a first-degree relative, it is not a given. Family genetics are not the only cause of Alzheimer’s. Known risk factors for dementia include obesity, chronic diseases such as type 2 diabetes and hypertension, smoking, and excessive alcohol consumption. A report from the Lancet Commission estimated that up to 40% of dementia cases could be prevented by addressing such risk factors. The Alzheimer’s Association cautions any person considering genetic testing should receive genetic counseling before a test is ordered and once results are obtained. Discuss with your healthcare provider about what is right for you.

Rheumatoid arthritis (RA) has a broad range of symptoms including joint inflammation and swelling as well as pain primarily in the hands, feet, elbows, and knees. Another common symptom is fatigue with seven out of ten patients saying they experience a lack of sleep and feelings of fatigue.

Doctors say there are several reasons why people with RA suffer from drowsiness and weakness. One of the main causes is a direct result of the inflammation that disrupts normal bodily functions. Another commonly cited reason reported by patients is a hard time sleeping through the night given the pain and discomfort they feel. 

The fatigue related to RA makes it hard to catch up on sleep. Without adequate sleep, people can suffer from a lack of concentration, and it’s harder to maintain an active lifestyle. Fatigue can also cause irritability and be associated with depression.

However, it is important to know that even though fatigue is common, doctors say if your symptoms worsen and/or you have a fever, persistent joint pain or unexplained weight loss, contact your healthcare provider. Experts say this can be a sign of disease progression or medical complications and require immediate attention. 

People with RA can help manage fatigue by eating a healthy diet and engaging in low-impact exercises. Practicing good sleep habits like adhering to a regular bedtime and avoiding alcohol and caffeine several hours beforehand can additionally help. There is some data suggesting limiting TV or your exposure to devices emitting blue light a few hours before bedtime can help you sleep better. 

Along with adopting good lifestyle habits, talk to your healthcare provider about medications to help reduce pain. Other strategies include learning relaxing techniques like breathing exercises and mindfulness and building a strong support network to reduce stress.

Normal aging brings about a decline in our physical abilities that affect strength, stamina and speed – all associated with declining muscle mass. Harvard Health Publishing explains that there is not much decline in our muscles between the ages of 20 and 40; after age 40 there can be a decline of up to 1% to 2% per year in lean body mass and 1.5% to 5% per year in strength. 

Mobility is key to protecting our independence. Along with losing muscle mass, other common problems in older adults include poor reflexes and balance, and loss of flexibility. Fortunately there are things we can do to encourage a healthier, safer aging process. 

One of the first things is to avoid a sedentary lifestyle. One prevailing myth many older adults believe is physical activity will lead to injury or a fall. Experts say the actual truth is too much sitting causes muscle loss and decreased flexibility. For older adults, it is more important to exercise regularly to compensate for bodily changes in hormones and other factors that occur with aging. 

Strength exercises using light weights or bands can help protect bones, build muscle and improve flexibility and balance along with aerobics, stretching and special balance exercises.  These include things such as brisk walking, jogging, biking, swimming, or cardio classes. To help build strength, balance, and flexibility, consider a class in yoga, tai chi, Pilates or isometric weightlifting.

Gingivitis – How to Prevent It & Treat It

Gingivitis is a common periodontal disease and can become a serious problem if allowed to go untreated. The disease attacks the tissue at the base of the teeth, called gingival tissue, and causes swelling and inflammation. It occurs when food particles and other substances are left on the gum line where sticky plaque and hard tartar forms at and below the gums.

Dr. Tien Jiang, assistant professor in oral health policy and epidemiology at the Harvard School of Dental Medicine, says with the correct oral care gingivitis can be controlled with proper teeth brushing and routine dental cleanings. If left untreated, inflammation from gingivitis can lead to an irreversible form of gum disease called periodontitis, and damage the tissue and bones supporting the teeth. If your dentist suspects periodontitis, they can use x-rays to see how far the disease has advanced. 

Studies have linked inflammation of the gums to a higher risk of many serious diseases including heart, stroke and dementia. Poor dental hygiene like not flossing, smoking, misaligned teeth, diabetes that is not well-managed, and infections in other parts of the body can put you at higher risks for gum disease.    

While early gum disease usually does not cause symptoms, as the disease progresses; signs include sore, red, sensitive, or swollen gums. Gums that bleed when brushed or flossed, mouth sores and bad breath are other symptoms. 

Dr. Jiang says prevention and treatment begins with home care and should include twice a daily brushing and once a day flossing. Pay attention to areas where plaque tends to grow such as between the teeth and at the gum line. If your dentist visits show lots of plaque buildup, they may recommend using anti-plaque or anti-tartar toothpastes or mouth rinses. 

At the dentist office, your dentist or dental hygienist is equipped to remove plaque deposits before gingivitis can cause more serious problems. For cases where inflammation is below the gum line, they have special tools to clean the area. Your dentist office is also a good place to learn how to properly care for your teeth.
