Understanding Dementia

Many of us, especially seniors, have heard or known someone with Alzheimer’s disease and have an idea of the devastation it causes. While there are other types of diseases categorized as dementia, Alzheimer’s is by far the most common form of dementia accounting for 60 to 80 percent of cases. You should know dementia is not actually a specific disease but refers to a decline in a person’s mental ability to such a degree it interferes with their daily life.    

It is also important to understand dementia is not a normal condition of aging but rather caused by damage to brain cells that affect a person’s memory, reasoning and other cognitive skills including an ability to communicate. There are several types of clinical dementia including: Alzheimer’s disease; vascular dementia; dementia with Lewy Body; and frontotemporal dementia. Mixed dementia is the term used when a person has both Alzheimer’s and another form of dementia, which is causing their mental decline. 

The Alzheimer’s Association says the “most common early symptom of Alzheimer’s is trouble remembering new information because the disease typically impacts the part of the brain associated with learning first.” As the disease progresses, the person is robbed of many other abilities such as speaking, swallowing and walking. 

To help get the proper diagnosis and rule out other causes that may be treatable such as side effects from a medication, it is important to see your doctor. If warranted, they may have you see a specialist that can do further testing.

Experts used to believe the brain could not make new neuron connections as we aged but research now shows the “brain remains nimble and capable of physical change and growth throughout life.” In fact, studies find people who step out of their comfort zone and try new hobbies and interests can improve their cognitive health. 

Another misconception is thinking it is too late to start exercising or to lose weight. A meta-analysis of 39 studies on aging and resistance-type exercises such as lifting weights or using exercise bands, found it is possible to build muscle mass no matter what age. In the studies,  “1,300 adults over the age of 50 who did some form of this kind of training, muscle mass increased by an average of nearly 2.5 pounds in just five months.”

Being positive is a trait that helps people deal more effectively with stress and is linked to longevity. Researchers also find it is better for us to be in charge of our own medical care. One way to do this is to stay up to date on medical research and advice regarding care options as well as asking pertinent questions of our healthcare providers. Diagnosing Alzheimer’s Disease

While there is not one single test used to definitively diagnose Alzheimer’s disease, doctors typically use a variety of tools to make a diagnosis. Along with the person’s primary care provider, specialists like neurologists, neuropsychologists, geriatricians and geriatric psychiatrists may all be involved in determining if the patient’s problems are Alzheimer’s or caused by other reasons.

The process starts with a medical workup done by a health care provider that includes the person’s history of medical problems as well as any history of psychiatric, cognitive or behavioral problems. It is important to tell the doctor about all current medications including over-the-counter-drugs and supplements the person is taking. Your doctor will also want to know if there is a family history of Alzheimer’s or other cognitive problems. 

A physical exam should include a check of reflexes, blood pressure levels and questions regarding sleep patterns, diet, exercise and other lifestyle habits. A structural brain imaging scan such an MRI or computed tomography is used to rule out damage to the brain from stroke, tumors, head trauma or a build-up of fluid. Blood and urine tests can rule out other reasons for symptoms. The Alzheimer’s Association says common causes of dementia-like symptoms include depression, untreated sleep apnea, delirium, side effects of medications, thyroid problems, certain vitamin deficiencies and excessive alcohol consumption.

The doctor may also use one of several cognitive, functional tests to evaluate the person’s memory, thinking and simple problem-solving skills. Professionals state, “There is an urgent need for a simple, inexpensive, non-invasive and easily available diagnostic tool such as blood tests to diagnose the disease.” Currently researchers are experimenting with a variety of tests to enable them to track changes more easily in the brain to identify the disease. Health professionals do not recommend routine genetic testing for Alzheimer’s disease at this time.

A Good Diet Promotes Healthy Aging

The National Institute on Aging (NIA) explains diet plays a large role in the aging process. To optimize our own health, they recommend we start by understanding the different food groups (published by the USDA); and how each one helps form healthy eating patterns for a particular age group. 

Experts say diet is one aspect of healthy aging and is especially important for seniors who are at greater risk for cardiovascular disease, cancer and diabetes as well as conditions related to loss of bone and muscle mass such as osteoporosis and sarcopenia. Other aspects of healthy aging include maintaining a healthy weight, exercising, and getting a good night’s sleep. 

One of the best food groups that encourage healthy aging is vegetables, which are rich in vitamins and minerals, and carbohydrates. These include dark green vegetables, red and orange vegetables, and starchy vegetables; as well as legumes (beans and peas) that are also found in the protein food group.  

The NIA reports fruits are another critical food group and are not consumed often enough by older adults. Fruits contain carbohydrates along with extra fiber that helps keep the digestive system moving. Recommendations include eating fruits with the skin on but be sure to wash them thoroughly before eating. If you choose to eat fruit in a can, dried or from frozen, choose options lowest in sugar.  

Fiber, a type of carbohydrate the body doesn’t digest, is another healthy food group. It can be found in vegetables, fruits, whole grains, and beans. Fiber-rich foods can help prevent stomach or intestinal problems and lower cholesterol.

At least half of our daily intake of grains should be consumed in products made with whole grains. These provide iron and many B vitamins. Examples include whole wheat bread, whole oats, whole bulgur (also known as cracked wheat), and whole cornmeal. 

For protein, it is recommended we choose low-fat meats, poultry, seafood, eggs, beans, nuts, seeds, and soy products. Dairy products help maintain strong bones and provide several vital nutrients including calcium, potassium, and vitamin D. Again, the best choices are low-fat options. Experts say also use oils instead of solid fats, such as butter or lard that are high in saturated fat whenever possible.

Helping Mom or Dad Transition to an Assisted Living Community

Moving can be exhausting and many of us have mixed feelings about the change – both looking forward to a new adventure and sad to leave our old life behind. Now imagine what your parents may be feeling as they leave a familiar home for an assisted living community. The change can be even more difficult if they are going into memory care. 

Experts say there are things family members can do to help ease the transition and make it a more positive experience. First, involve your loved one into the decision-making process as much as possible. Discuss the various benefits of each community regarding suites, amenities, food service and the helpfulness of the staff and administrators. Also, read online reviews from other families and residents; and take advantage of in-person tours.

Once the decision is made on a particular community, now help mom or dad decide on what to bring to their new space, keeping in mind it is probably smaller than their current home. Consider an item based on its practicality, footprint, and your loved one’s feelings toward it. Be sure to include personal mementos such as family pictures and a few of their most cherished treasures he/she has collected through the years. Talking About End-of Life Issues With a Loved One

Medical professionals and counselors who deal with families facing end-of-life issues offer a few suggestions to the rest of us on “how to start a conversation” when we have a very sick loved one. They say the first thing to consider is having the conversation in one’s kitchen is a whole lot easier than having it in the hospital. It may also encourage your family to start the conversation if they learn that studies find families who have talked about it beforehand express less feelings of guilt after the person passes away. Moreover, they say they had an easier time grieving and were less likely to experience depression. 

The type of questions to ask to get a conversation started include “what does the person value most about their life?” Other topics of discussion could be to find out what the person understands about their illness; what matters most to them at the end of life; how they want to handle medical decisions; and what their fears and physical needs are. 

As a family member, you need to remember to practice active listening. It is important to leave your own views out of the discussion and try to understand what the person is feeling. Find out if they want to spend their last days at home; and who do they wish to be surrounded by as they face death. This is also a good time to encourage them to express their wishes regarding burial. 

Ask if the person has an advance directive, which is a legally binding document that gives instructions for healthcare if they are no longer able to make or communicate those decisions themselves. This can include their wishes around discontinuing treatment and medications; and saves the family from having to make difficult decisions.

Women and Bone Health

Endocrinologist Kendall Moseley, MD, medical director of the Johns Hopkins Metabolic Bone & Osteoporosis Center, says most women do not know anything about their bone health. A recent survey from the National Osteoporosis Foundation backs up that sentiment finding around 94 percent of women do not worry about their skeletons. However, they add they should, as one in five women under 30 already have osteopenia, which is a step above full osteoporosis. The good news is there are things you can do to protect your frame and avoid the things that are not good for bone health.  

Experts say our bones constantly break down and rebuild every 10 years resulting in a new skeleton. The problem lies in the fact that after the age of 30, our bodies start to lose more bone than they make. Women’s estrogen levels also begin to drop around 30, which is another protector of women’s bone health. 

An unbalanced diet or severely restricting calories in an effort to stay thin or lose weight can be another cause of low bone density; and too much caffeine, salt and alcohol all interfere with retaining calcium, our primary bone builder. Even some medications deplete calcium and include certain anti-depressants and contraceptives so be sure to ask your doctor about side effects of medications you take. It is important for women to include dairy products or foods like beans and nuts to increase their calcium intake. Protein from plants and lean meats are also good ways to get calcium.

Along with a good diet, exercise is an important element of keeping bones strong. While some exercises are better than others, scientists recommend weight bearing exercises like walking and running as good ways to build strong bones. Strength training exercises may be the ultimate bone builder and include things like pushups and squats or kickboxing.

Concerned About Memory Problems?

Since it is true our brains may not function as well as they did when we were younger, not all brain changes are a sign of Alzheimer’s or other dementia-related diseases. It may simply be a sign that as our bodies move slower with age, so do our brains.

An AARP article highlighted memory problems that can be a part of normal aging as well as problems that should be brought to the attention of your healthcare provider. One example discussed was the difference between getting lost or confused in a familiar location as opposed to getting lost the first time you go to a new place. Experts say the former is concerning and could be a sign of a problem as it goes beyond normal aging. Also, if you find yourself forgetting where you are at times or how you got there, tell your doctor.

Another worrying sign can be repeating the same story or asking the same questions over and over again. The person probably doesn’t remember repeating themselves; if they did, they wouldn’t do it. This is why if a spouse or friend tells you are constantly repeating yourself, add it to the list to mention to your doctor. 

Forgetting appointments can be normal when it happens occasionally but concerning if it happens all the time. Moreover, there is a difference between forgetting a planned activity until reminded about it, or forgetting the activity was never planned in the first place. Other signs it could be dementia include not being able to complete tasks you used to do or not being able to multi-task like you once did. 

Normal forgetfulness includes things like forgetting a person’s name and then remembering it later; or not sure what day it is but later it comes to you that it is Sunday. In addition, as the brain’s processing speed begins to slow down, it is normal to take a longer time to learn something new.

The Importance of Arm Strength for Older Adults

We know balance exercises protect older adults’ safety and stability by preventing falls as well as helping maintain independence. The Harvard Medical School explains keeping arm strength should also be a goal for seniors. 

The fact is as early as our mid-thirties, arm strength and overall muscles start to decline and continue to do so around one percent each year thereafter. Once we reach our sixties, experts say arm strength is declining by as much as 3 percent each year, and not only reduces our range of motion but increases our risk for injuries. 

Functional strength improves quality of life, and having a strong upper body is important. Maintaining and/or building up arm strength benefits our bones and balance as well as prevents falls. Experts point to two important muscles that control our arm movements: biceps, the muscles located on the upper arm; and triceps, the muscles at the back of the upper arm. Both sets of muscles help us lift, carry and push. 

Improving arm strength is easy to do at home and doesn’t require fancy gym equipment. Check with the Harvard Medical School or other sources on the Internet to find examples of exercises targeted toward seniors. To get started, all you need are a pair of two or five pound weights depending on your size and/or condition. As you gain strength, you can move up to heavier weights. However, before beginning any new exercise, please check with your healthcare provider.

Why Balance is Important to Your Independence

Falls among senior citizens are all too common, and in many cases, threaten a person’s independence and way of life. Injuries from falls include to the head, hip fractures, and in extreme cases lead to death; and poor balance is one of the reasons people fall.

Experts explain a person’s balance declines with age due to loss of sensory elements, the ability to integrate information and issue motor commands; and loss of musculoskeletal function as well as a poor vision and inner ear problems. Certain medications and common medical conditions in older adults can also cause problems. Balance begins to decline in midlife, starting at about age 50. In one recent study, adults in their 30s and 40s could stand on one foot for a minute or more. By age 50, the time decreased to 45 seconds.

The National Institute on Aging (NIH) explains maintaining good balance as you age and learning about fall prevention can help you get around, stay independent, and carry out daily activities. Targeted exercises like walking, strength training and workouts designed to improve stability are some of the best ways to improve balance. Consult with your doctor as to whether a physical therapist or other professional could help design suitable exercises to improve your balance. 

Also talk to your doctor about any dietary changes you should make if your balance problems are due to high or low blood pressure. Something as simple as eating less salt, maintaining a healthy weight and exercising may help with high blood pressure. 

If you have low blood pressure and experience dizziness, do not stand up too quickly, drink plenty of fluids such as water and avoid alcohol. Your doctor may need to evaluate if it is safe for you to drive. To reduce your risk of a fall, avoid walking in the dark, wear low-heeled, rubber-soled shoes; and if you need a mobility aid such as a cane or walker, use it.

Scientists say in reality old age is probably more varied and modifiable than most of us think. In fact studies show the leap from middle age to late life has no definite biological tipping point. Eric Verdin, president and CEO of the Buck Institute for Research on Aging in CA, reports, “There is an incredible amount of variability between different people.” His research has convinced him that a person’s biological age is more important than their chronological age. Recently the journal of Psychology and Aging published findings that support pushing up the number of what is considered old age. 

As people start to live longer, they want to postpone the age in which they are considered old, which many see as a negative condition. In fact experts say old age stereotypes have steadily become more negative over the last 200 years and have fueled “what some call a worldwide crisis of ageism.” This can be problematic as studies find even one’s beliefs regarding old age can lead to worse physical, mental and cognitive health. 

While we do know everyone ages differently, it has brought a group of people known as “superagers” to the attention of researchers who want to study why some people remain relatively youthful and healthy long after 70. They speculate one reason may be studies that show adults with a better attitude toward aging are often healthier than their peers.

Death is part of life and if we are fortunate to live a long one, most likely we will have to deal with the loss of a beloved spouse, life partner, child, sibling or friend. Grief counselors suggest some things a person’s family and friends can do to help them get through the worst of the grief. If it was your parents who died, be sure to let your surviving parents know how much you miss dad or mom, and share memories you have of the good times. Ask your parents if they have more difficult times during the day dealing with the loss as you could come over or give them a call. The Centers for Disease Control and Prevention (CDC) reports 3 million seniors are treated in emergency departments for fall injuries each year. Furthermore, more problems can arise as many seniors need several months to recover, which can cause a decline in strength and mobility as well as mental agility.  It can also lead to the loss of independence and isolation. 

Statistics show most falls happen for a handful of reasons and being forewarned may help prevent one. The environment, especially the person’s home and surrounding outdoor area is one of the biggest culprits. Danger spots include slippery bathtubs, high mattresses, steep stairs, poorly lit hallways, loose rugs, obstacles in walkways and clutter. Experts say a little bit of time spent addressing these problems with things like non-slip bath mats and grab bars, more lighting, removing clutter, etc. can help prevent a fall. 

Another risk factor for a fall is chronic health conditions that affect balance and mobility. Arthritis, heart disease, diabetes and high blood pressure can mean a person might experience dizziness or be unable to control muscles to stop a fall. 

Growing older also puts us at risk for diseases like glaucoma, age-related macular degeneration, diabetic retinopathy and cataracts, which in turn, leads to more accidents. Solutions include having yearly eye exams and making sure your eyeglass prescription is up to date.

Experts say it is important for seniors to stay as fit as possible as aging affects muscle strength, flexibility and coordination. Check with your doctor for information on balance and strength exercises. If a medication is making you dizzy or drowsy, talk to your doctor about it. There may be another drug just as effective that doesn’t have the same side effects.
