Are Memory Problems Interfering With Your Life?

Do you feel as if memory problems are interfering with your life or have family members started to notice? Medical experts say there may be a distinction between a person noticing memory loss and others becoming aware of it as this could signal a bigger problem and be an early sign of dementia.

The Harvard Medical School explains the best place to start understanding the problem is to talk to your doctor. While they may not have the proper training to do extensive screening for MCI (mild cognitive impairment), they can rule out other things causing your memory loss such as medications you take or a recent head injury. Steps you can take on your own to improve cognitive function include getting enough sleep each night, eating a healthy diet, exercising regularly, and reducing stress.

After your own physician has completed an examination, and nothing appears to explain your memory loss, it is time to see a specialist. They, or your local Alzheimer’s Association chapter or hospital should be able to recommend a behavioral neurologist or a geriatric psychiatrist. He or she will probably start with a series of written, on the computer, or oral tests to “assess memory, reasoning, attention, language, visual functions, motor functions, and social functions (such as empathy and knowing how to behave appropriately in social situations).”

They will check for “abnormalities in muscle strength, coordination, reflexes, senses, eye movement, and the pupils' reaction to light, which can signal abnormalities in specific areas of the brain.” In addition, the doctor will check for Parkinson’s, a stroke or tumors, and will probably schedule a hearing and vision test. If indicated, you may have advanced testing such as PET scans or your cerebrospinal fluid (CSF) biomarkers checked for signs of Alzheimer’s disease.

An advanced directive is a legal document giving you a voice involving medical decisions in the event you are incapacitated. Many people shy away from creating such documents as they fail to realize how important they are, and mistakenly worry they are signing away their lives, and/or don’t like thinking about being so ill they are unable to make decisions.

Advance care planning documents may include a living will, power of attorney and health care proxy. By planning ahead, they guide decisions made on your behalf if you are unable, and suffer from an incurable, irreversible illness, condition or disease determined to be terminal. Living wills specify the kind of medical treatment desired and many simply express the desire to not be given life-sustaining measures that only serve to prolong a person’s death. Specific details may regard care including the use of pain medications, antibiotics, hydration, feeding and the use of cardiopulmonary resuscitation or ventilators.

A health care proxy is someone designated to express your wishes for medical care and has the same rights you would in requesting or refusing treatment. A durable power of attorney document names a person to act on your behalf if you are incapacitated and gives them the power to make financial decisions.

The Harvard Medical School reports on some of the myths people have regarding advance care directives. They say you need to realize more care is not always better but can prolong the dying process without respect for quality of life or comfort. Refusing life support does not mean you are committing suicide but rather allowing your medical problem to be the cause of your death. Moreover, refusing life-extending treatment does not mean you are refusing comfort measures for pain management and symptoms. Talk to your family and doctor about your medical preferences, and be sure each has a copy of your advance directive documents.

As Alzheimer’s disease progresses, a person’s needs change and it is important to recognize and address those changes with appropriate care. The Alzheimer’s Association says determining the best care options can be tough and offers some advice on how to ensure the person has the services, programs, and caregivers in place for the best possible quality of life.

The first step to any decision regarding care is to honestly evaluate your loved one’s needs. Also, to whatever degree possible, it is recommended you include them in any decisions. Another thing to remember is during the early stages of the disease the person may be able to live independently but in the middle stages, 24-hour supervision is needed. Additionally, around-the-clock care is much more demanding in the later and end stages.

Primary points to consider include safety, especially in terms of unsupervised cooking, or if the person has a tenacity to wander or needs help with medications. Does the person have medical conditions that need monitoring such as diabetes or a mobility issue? Other problems that may demand more professional care include a person becoming aggressive or needing help with the ADLs (activities of daily living). Finally, consider if the person would benefit from more social interactions and activities.

Snacks Should be a Balanced Addition to Your Daily Nutritional Intake

The goal of a snack should be to satisfy your hunger as well as be a balanced addition to your daily nutritional intake. Dietician Lisa Jones says satisfying snacks can be crunchy, creamy, savory or salty and good choices include things like eggs, tofu, whole grains, and fruits and vegetables. 

As a guideline, an adult’s snack should contain approximately 150 to 250 calories. Experts advise diabetics to stick to protein-rich choices and to watch the amount of carbohydrates a food contains as carbs can elevate blood sugar. If you are eating pre-packaged food, check to see what constitutes a serving and the number of calories.

Remember healthy snacks can be tasty such as avocado on whole grain toast. For a fiber-rich snack, how about making chia pudding? The recipe is simple and all you need to do is add 3 tablespoons of chia seeds to a 6 oz. glass of seed-based milk. Add ½ cup fruit and refrigerate for a couple of hours. To get 80% of the daily folate and 50% of the manganese requirement, how about eating a cup of shelled edamame?

Other quick snacks include a handful of nuts and a piece of fruit, or a bowl of popcorn topped with a small amount of olive oil rather than butter. Cut-up veggies eaten plain or dipped in peanut butter or humus is another satisfying, healthy snack. Greek yogurt is loaded with calcium, probiotics and protein and can be added to fruit, a handful of nuts or even to tuna.

Sleep Apnea is a Serious Condition

The US News & World Report says approximately 1 in 3 adults are believed to have obstructive sleep apnea (OSA). They explain the condition causes a person to momentarily stop breathing at intervals throughout the night and interrupting their sleep.

Pulmonologist and section head of sleep medicine at National Jewish Health in Denver, Dr. Sheila Tsai, explains, “A person’s airway either partially collapses or completely closes (obstructs) during sleep.” She says episodes can be frequent throughout the sleep cycle and cause drops in oxygenation. While the person may only awake briefly as they struggle to breathe, their sleep is fragmented and non-restorative as it infers with REM, the deep, restful stage of sleep.

Common causes of OSA are obesity and age, and The Journal of Clinical Sleep Medicine estimates as many as 80 percent of people suffering from OSA are undiagnosed. The condition typically causes the person to snore loudly, interrupted with gasps, hesitations and snorts. They may wake up feeling unrefreshed and sleepy during the day, or complain of headaches. OSA can additionally cause other sleep disorders to become worse like restless-leg syndrome and sleep walking.

Doctors say OSA is a serious, but treatable condition and it is important to seek medical care. A sleep study or test done in a clinic setting or even at home is the best way to diagnose the condition. Untreated OSA can put a person at a higher risk of heart disease, stroke, diabetes and other problems.

When to See a Doctor About Memory Loss

Growing older can bring with it a variety of problems including worrying about your ability to remember things or the fact that it now takes you a lot longer to learn something new. While you may be concerned about forgetting where you put your glasses or the date of your next doctor visit, or even forgetting to pay an occasional bill, experts say most likely it is just normal aging.

So, what are the differences between normal forgetfulness and signs of a deeper problem? The National Institute on Aging (NIA) says it is about the degree of forgetfulness. While mild forgetfulness may be normal, memory loss that impacts your day-to-day life is a cause for concern and includes things like forgetting how to get home, or problems driving or using your phone. 

The NIA says this is the time to have an honest discussion with your doctor as he/she can help determine whether it is normal aging or something else that is causing the problem. If indicated, they can do a series of cognitive and medical tests to narrow down the problem. It is important to remember not all memory loss is a sign of Alzheimer’s disease or another type of dementia but may be the result of a medication you are taking, depression, infection or a medical problem. 

Doctors say it is important to get a diagnosis because if it is dementia there are medications and treatments that can help. A diagnosis of mild cognitive impairment (MCI) means a person is considered to have more memory or other thinking problems than other people their age. If a person does have MCI, typically they can still care for themselves and do their normal activities. While it can be an early indicator of Alzheimer’s disease, the NIA says not everyone with MCI will develop Alzheimer’s disease.

Grieving for a Loved One With Alzheimer’s Disease

When a loved one is diagnosed with Alzheimer’s disease, it is devastating news for all. Family members may quickly start experiencing feelings of grief as they recognize their life is about to be changed by the disease and are uneasy about what the future will hold. Most are left with questions about what to expect next.

As Alzheimer’s begins to take away the person we know and love, the Alzheimer’s Association explains that, “you'll mourn him or her and may experience the different phases of grieving: denial, anger, guilt, sadness and acceptance.” They remind us that feelings of grief do not come in a specific order but rather we go in and out of the various stages.

Feelings of denial include unrealistically hoping the person is not ill or expecting them to get better. Others try to convince themselves their loved one has not changed or try to normalize and make excuses for their behaviors. Anger may include being frustrated with the person or feeling abandoned. You may also experience feelings of resentment with the demands of caregiving and/or resent others who are not helping with care.

Guilt includes having unrealistic thoughts about what you should have done or feeling guilty if you do not visit every day. A spouse that is well and still able to enjoy life may feel guilty that their husband or wife is no longer able to do so. Others may have negative feelings such as wishing the person would die or go away.

The Alzheimer’s Association says you need to face both your positive as well as negative feelings. They also say to be prepared to go through feelings of loss more than once as the disease progresses. Remember your grief is a part of your journey, and if you need help, talk with someone about your feelings.

Taking Prescription Medications Safely

Every year seniors are hospitalized due to prescribed medications. Statistics show drug-related problems are common in older adults and include drug ineffectiveness, adverse drug effects, over-dosage, under-dosage, inappropriate treatment, inadequate monitoring, nonadherence, and drug interactions. It is important to remember even though medication is prescribed by your healthcare provider, problems can occur.

Experts say you must follow directions and take medications as prescribed including on time, in the right dosage, with or without food, and never grinding it into a powder (unless directed by the doctor). They recommend carrying an updated list of all medications and sharing it with your doctors. Additionally, be aware of common interactions between prescribed drugs and over-the-counter medications and alcoholic drinks.

Before your doctor prescribes a new medication, ask them why you are taking it, for how long and what are the potential side effects. It is important to know what to look for and to discuss the benefits versus any negative or dangerous consequences. You will want to know, for example, if a drug will make you drowsy so you know not to drive under its influence.

Another thing to safeguard your health is to review your medications with your doctor on a regular basis asking questions such as: do you still need to take it, is the dosage correct, and is there a non-drug alternative? Despite your faith in your medical team, it is not a bad idea to check your medications against the American Geriatric Society’s “2023 American Geriatrics Society Beers Criteria for Potentially Inappropriate Medication Use in Older Adults.” The online site lists medications that may be inappropriate or dangerous for seniors.

Do You Need a Mobility Aid?

Many older adults, especially those in assisted living communities, are no longer as steady on their feet as they used to be.  This puts them in danger of a fall, and a walker, cane or even a wheelchair or scooter is probably advisable.  However, before you start using a hand-me-down walker or buy any-old cane; experts warn that one size does not fit all.  In the case of a walker, to be an effective and safe mobility aid, it is important to have an expert help you pick out the best type and size for your needs.  

As AgingCure.com points out, “The wrong device can negatively affect a senior’s stability, cause posture-related problems and even result in pain. Mobility aids come in all shapes and sizes, so selecting the right equipment and achieving a proper fit usually requires a professional’s assistance and expertise.”  To get started determining what your needs are, experts recommend seeing your healthcare provider to discuss any problems you are having with your activities of daily living (ADLs).  If they recognize a need for a mobility aid, they may refer you to an occupational or physical therapist.  

The following are a couple of basic guidelines to know before you get started.  A cane can help provide balance and stability and is ideal for someone who needs to support no more than 25% of their body weight.  It is most useful for those minimally impaired or with weakness in one leg.   Walkers are available in a variety of models but regardless of the style a “correct height of a walker is measured from the floor to the wrist, and the elbows should be flexed in a range of 15 to 20 degrees.”  Experts also say the walker must be large enough to provide front and side support for the user.  

A motorized scooter may be the answer for someone who has lost the ability to walk any distance but still has an adequate amount of strength in their arms and upper body to stay upright as well as drive the scooter.  Again, there are a variety of models including indoor and outdoor ones as well.  Keep in mind though that most insurance companies and Medicare Part B typically only cover scooters for in-house use.
