Learn More About Mild Cognitive Impairment

Studies report approximately 12-18% of people aged 60 or older have some level of Mild Cognitive Impairment (MCI). So how do you know if you are one of them? The first step to diagnosing MCI typically comes after a person starts to complain of forgetfulness or cognitive problems or initiated at the urging of family members. Doctors explain it is usually more serious if it comes at the request of those closest to the person because as a person develops more of the signs of Alzheimer’s disease, they are often not aware of their memory loss. So, how is MCI diagnosed?

Your doctor will begin with a thorough evaluation to try and determine if the problem is MCI, Alzheimer’s, normal age-related memory loss, or caused by another factor. It is important to note not all cases of MCI develop into dementia and an evaluation could identify medications as the problem. A Harvard Medical School article states, “Establishing the cause and scope of the impairment is helpful in setting a baseline for future evaluations to determine if the condition is progressing to dementia.” 

Some of the preliminary tests include a complete blood workup to rule out anemia, infection, kidney and liver disorders, or diabetes. Other tests may check for thyroid abnormalities, elevated blood calcium and vitamin B12 deficiencies. 

Cognitive tests begin with an in-depth medical history as well as questions regarding the person’s memory lapses and other thinking problems. Your physician will probably request a family member be there to reaffirm and document the changes. Experts say quickly progressing memory problems are most likely another issue as true dementia progresses slowly. 

Your physician may refer you to a neuropsychologist for more tests including paper and pen and computer tests to assess memory, language and other cognitive skills including motor and visual function. If deemed necessary, they may do structural and functional brain scans or an amyloid PET scan, which detects in a living brain the signs of amyloid plaques, the hallmark of neurodegenerative diseases like Alzheimer's.

Ideas to Celebrate the Special Moms in Your Life

This year, Mother’s Day falls on Sunday, May 8th.  Rather than sending a bouquet of flowers this year, show grandma, mom and other moms in your life your appreciation and love by giving your time. 

Do something special like planting a new rose bush or a tree, putting in a tray of annuals, or cleaning up the garden in readiness for summer. Commit to helping mom with a DIY house project like painting a room or helping clean up the attic or basement. Give mom and grandma the “day off” by taking over the cooking and cleaning up. For a simple dessert, set up a sundae bar with all the trimmings and let everyone make their own treat. 

If mom lives in an apartment, decorate her front door with a wreath or a huge homemade card signed by the whole family telling her how special she is. For a movie-loving mom, make a date to watch one of her favorite romantic comedies together in person or virtually. Other fun ideas include hosting a garden tea party or barbeque, and if you can’t get together, send mom all the goodies you want her to enjoy at a virtual party.

Spend time together baking a fancy dessert or cooking a family favorite recipe for dinner. If mom lives close by, put together a basket of homemade goodies and deliver them to her door. The grandkids can help grandma feel loved by making cards or putting on a talent show or play. Researchers say despite declining cognitive abilities, studies show those with Alzheimer’s disease do better when encouraged to participate in meaningful activities as it gives them a sense of purpose and self-worth. The Nation Institute of Health (NIH) reports, “engaging older persons with dementia in appropriate activities has been shown to yield beneficial effects such as increasing positive emotions, improving activities of daily living (ADL) and improving the quality of life.” One study’s author explains the act of staying engaged can help dementia patients maintain quality of life even in advanced stages of the disease. 

Just as it is important for older people to stay engaged and active, it is also important for those with Alzheimer’s and other forms of dementia to be involved in healthy daily activities. Dementia care specialists say structure and routine helps individuals “maintain their cognitive function, sense of security, and can calm anxious or aggressive behaviors.” Especially in the early stages of the disease, these types of activities give a person a sense of control over their day. Toward the latter stages of the disease, purposeful activities can be one of the few remaining ways to engage in memories and communicate.  

Studies show the creative arts are an effective way to stimulate the five senses. Singing has been shown to not only be enjoyable to those with dementia, but often is one of the last abilities they lose. A person may remember the words to a song long after other memories disappear. Dancing to or listening to popular music from their youth is another activity shown to stimulate the person’s senses and increase self-awareness, while reducing anxiety and depression. Painting or drawing can do the same for others. Experts say don’t forget the value of exercise. A short walk outside can lift spirits but make sure the person is steady on their feet as Alzheimer’s can affect gait and balance.

Breakthroughs in Alzheimer’s Research Offer Hope for the Future

Recently John C. Morris, MD, Director of the Knight Alzheimer Disease Research Center at Washington University in St. Louis and member of the ABF research advisory committee, said there are several reasons to have hope and optimism for the future regarding Alzheimer’s disease. Involved in Alzheimer’s research since 1983, he explained for the first time he believes “there is now a great opportunity to develop effective therapies, including drugs currently under evaluation for treatment.” He said advances also include an important tool for diagnosing the disease.

One of the reasons for optimism was the announcement of the pharmaceutical company Biogen’s controversial new drug, which purportedly is designed to treat Alzheimer’s disease and not just the symptoms. The drug, aducanumab, was approved for patient use on June 7, 2021. However, doctors say it is important to know the FDA’s approval is only for the drug’s ability to reduce the amount of amyloid plaques in the brains of people given the drug. They state it is still unknown whether the removal of the plaque offers benefits to the patients. To be eligible to receive the drug, patients need to have a firm diagnosis of Alzheimer’s disease.  

In the year 2020, we also heard the news about a ground-breaking new blood test at the Alzheimer’s Association International Conference 2020. Researchers say the new test identifies the protein ptau-2017, a biomarker for Alzheimer’s, and results have been far more accurate than several other biomarkers currently being tested. This is good news as doctors say earlier testing will lead to earlier interventions. The test received a Breakthrough Device designation by the FDA in October 2021.

Add in a few boxes of single-serve cups to use with the maker including hot chocolate or flavored coffees in a selection they would enjoy.

The CDC Estimates One Million People are Diagnosed with Shingles Each Year

Currently Shingrix is the only vaccine approved for adults 50 years and older to protect against shingles. This vaccine is important as the Centers for Disease Control and Prevention reports nearly one in three people in the U.S. develop shingles at some point, and every year an estimated 1 million people are diagnosed with the disease.  

Symptoms of shingles include pain, rash, itching, and at first fluid-filled, then eventually crusting blisters. People can additionally suffer from a hypersensitivity to touch or light as well as headaches and fevers. The disease can last anywhere from a few weeks to several months and pain is generally most intense in the first few weeks. Some individuals may require pain medication to sleep or get comfort.   

When Shingrix first came on the market in 2017 so many people wanted it there weren’t enough dosages to meet demand. Now manufacturer GlaxoSmithKline reports there is plenty of supply available for anyone who wishes to get the shot. For many people who put off medical care during the COVID-19 pandemic, now may be the time to protect yourself against shingles.

The vaccine requires two doses given any time between two to six months; and studies show it to be highly effective against the shingles in both older healthy adults and frailer ones. “In a large multicenter study of approximately 27,000 people vaccinated with Shingrix or a placebo, effectiveness ranged from about 90% in frail participants to nearly 96% in non-frail participants.”

To Begin Exercising Again, Stretching is a Good Place to Begin

If you are senior who wants to start exercising again, you may not know how to begin.  For one thing, you probably realize you are not as fit as you used to be, and are not up to the same degree of physical activity that you once did.  And of course, before you begin any new form of exercise, you will need to check with your doctor to make sure you are fit enough, especially if you have osteoporosis, an injury, or balance issues.  

Once you get the go ahead from your healthcare provider, stretching is a good place to begin.  In fact, studies show that as we grow older our flexibility decreases by up to 50 percent in some joints.  Another good thing about stretching is many exercises are appropriate for any level of fitness including ones that can be done sitting in a chair.  Stretching can help you maintain your flexibility and mobility as well as loosen up some of your stiffness in your back, neck and shoulder muscles.

Another plus about stretching exercises is that most of them do not require special equipment and can be done anywhere.  The Silver Sneakers website recommends that as you stretch, “to breathe deeply, and go slowly. Listen to your body, and never force a movement that causes pain.”  They remind seniors it is okay if they cannot bend very far as over time they will loosen up some.  Safety is always important and you do not want to hurt yourself.  Other exercises especially good for seniors include chair yoga, resistance band workouts, walking, and dumbbell strength training.

In an article from Medical News Today, experts explain the various areas dementia research is focused on including: “understanding how the condition works, why it develops, and how it might be treated. Importantly, there is also a focus on how we can improve the lives of people with dementia.” 

Dr. Kellyn Lee, a chartered psychologist and research fellow in aging and dementia at the University of Southampton, in the U.K., is one researcher focused on trying to improve the lives of those suffering from dementia. Her program, called the “Material Citizenship Framework Project,” is being used to demonstrate to staff working with dementia patients just how important patients’ possessions and identities are to their wellbeing. Dr. Lee says in one case an Alzheimer’s patient was distressed at not recognizing herself. It turns out before her illness; she dyed her hair and didn’t recognize the woman in the mirror with gray hair. Personal possessions can help people to connect with and help maintain their identity. Dr. Lee explains, “Losing access to frequently used possessions can cause disempowerment and confusion. To observers, this confusion can sometimes seem like dementia progression.”

Another group of researchers are working to improve the lives of people in the advanced stages of Alzheimer’s with the development of HUG comforters. These soft blankets have weighted limbs, a heartbeat and the ability to play the person’s favorite music. Studies show a HUG can help reduce anxiety and agitation, and embrace the person in a soft hug. 

In the on-going race for effective treatments for Alzheimer’s, some researchers are looking at repurposing certain drugs. One example is a drug used to treat type 2 diabetes. Metformin has shown to improve symptoms in Alzheimer’s patients in a small clinical trial but more studies need to be done. Others are studying the connection between cardiac disease and Alzheimer’s and how decreased blood flow adversely impacts our brains.

The U.S. News and World Report published an article on tasty treats designed to satisfy your sweet tooth and provide a healthier alternative to more sugar laden desserts. One suggestion from board-certified oncology dietitian, Kailey Proctor, of the Leonard Cancer Institute at Providence Mission Hospital in Mission Viejo, California, is to grill pineapple, banana, apple or pear slices. Grilling concentrates flavors by reducing the water content, and further sweetens the fruit as natural sugars are caramelized by the heat.  

Dark chocolate is a known powerful source of antioxidants as research shows it can help improve blood flow, lower blood pressure, and raise HDLs, reducing your risk for heart disease. One healthy, easy-to-make dessert is to melt dark chocolate and spread it over parchment paper. Then sprinkle dried cherries, chopped nuts or sunflower seeds over it; refrigerate for five minutes and cut into small pieces. Another variation is to dip fresh strawberries or other fruit into melted dark chocolate.

Yogurt parfaits are fun to put-together and can be customized to your taste. Using a parfait dish or small bowl, add granola, strawberries, blueberries, nuts, peanut butter or seeds, layering with yogurt. To cut down on fat, use nonfat or low-fat yogurt as it still provides a good source of protein without the calories.

How about banana ice cream made from frozen pureed bananas mixed with almond milk? If you like, add in peanut butter and nuts. Sprinkle some cinnamon on top, which is “thought to have anti-inflammatory effects and may help diabetics manage their blood sugar levels.”

Reducing the Risk of Falls

The Centers for Disease Control and Prevention (CDC) reports that each year, one out of every five Americans aged 65 years and older has a fall that results in an injury such as broken bones or a head injury.  However, medical professionals say while it is important for seniors to take precautions to prevent falls as injuries can be serious, they also remind us that regardless of how old we are, it is beneficial for our overall health to keep moving.  Experts explain that failing to engage in “mild exercise on a regular basis results in reduced muscle strength, decreased bone mass, loss of balance and coordination, and reduced flexibility.”

They go on to say that despite commonly held beliefs, falls are not a normal part of aging and there are things you can do to help keep you on your feet and mitigate the risk of a fall.  Their first advice is to talk to your doctor about your risk and prevention.  Tell them if you have fallen and discuss your medications to see if side effects include dizziness or sleepiness that could contribute to falls.  

While falls are common in seniors 65 years of age and older, individuals with dementia are at an even greater risk.  Researchers say there are several reasons for this including gait and balance issues as well as problems concentrating and with staying focused on the task at hand.  Other issues that may increase the risk of falls include poor eyesight and chronic diseases such as Parkinson’s, and arthritis or numbness in the feet.  If you often feel dizzy or lightheaded upon standing or suddenly sitting up, ask your doctor if you might have postural hypotension, and find out how you can alleviate the symptoms.    

Further tips to reduce your risk include having your eyes checked annually and updating your glasses when needed.  In addition, talk to your doctor about the best footwear for inside the house as well as outside.  Check your home for trip hazards (the CDC has a list of tips) and install grab bars in the bathroom.  Finally, ask your doctor about exercises to strengthen your leg muscles as well as balance-training exercises to improve balance.

Encouraging them to stay involved in meaningful activities is a good way to give them a sense of self-worth and purpose.  In fact, a published report from the National Institute of Health states, “engaging older persons with dementia in appropriate activities has been shown to yield beneficial effects such as increasing positive emotions, improving activities of daily living (ADL) and improving the quality of life.” It goes on to say it is important for those with dementia to stay as active and engaged as possible and that “it may even help to maintain quality of life in the more advanced stages of the disease.”  

As to what kinds of activities are the most effective, studies suggest trying to stimulate all five senses: sight, touch, sound, taste and smell; and the creative arts are a good place to start.  Singing is not only enjoyable but several studies have shown it to be one of the last abilities many of those with Alzheimer’s lose.  People can often remember the words to a song long after many other memories are gone.  Listening to music and dancing are other activities beneficial to those with dementia.  Dancing has even been shown to help stimulate social interaction, enhance mood, reduce anxiety and depression, and increase self-awareness and self-expression.  Walking and getting outside can also lift spirits, but as gait can be adversely affected by Alzheimer’s, evaluate the person’s ability to walk safely.
