Cataracts are cloudy areas that form in the lens, which is normally transparent, and are the main cause of vision loss in people over 40 years of age. It is estimated more than 40 percent of Americans aged 75 years or older have some degree of lens clouding; and over half of people 80 or older have their vision significantly impaired by cataracts or have had cataract surgery. Surgery is generally recommended once vision is impaired to a degree that people have trouble reading, driving and recognizing faces. Today the surgery is a quick, safe procedure, replacing the cloudy lens with a clear plastic one. 

Doctors say there are things people can do to reduce their risk of developing cataracts with quitting smoking at the top of that list. Medical News Today reports on studies linking a 3-fold increase of smoking to cataracts as well as raising the risk of a number of other eye conditions. A healthy diet of fresh fruits, vegetables, whole grains, good quality fats (avocados, olive oil, and omega oils) and lean animal-sourced or plant-sourced proteins additionally reduces the risk of cataracts. 

Maintaining a healthy weight is important as obesity increases your risk of developing type 2 diabetes, which is a risk factor for cataracts. Other ways to reduce your risk include wearing UV blocking sunglasses when in the sunlight and getting at least 7 hours of sleep each night. The risk for cataracts also increases with age, family history, and a history of eye inflammation or injury. 

If you are not ready for surgery yet, doctors say stronger glasses and brighter lights may help. Keep up with regular eye exams and renew your eyeglass prescription as needed. Wear sunglasses in the sun and avoid night driving. As your cataracts continue to develop and eyesight worsen, at some point you will most likely need surgery.

In fact, studies show ADL disabilities are associated with an increased risk for mortality; and taking care of daily needs can help prevent new and worsening health problems.” They also report if family members notice a decline in a loved one’s overall health, taking steps now can improve their safety and forestall further problems. Medications That Increase Fall Risk

The Harvard Medical School reports it is estimated 1 in 3 people over age 65 will fall each year, and every 19 minutes a senior dies from a fall-related injury. Statistics show women are at higher risk for a fall but men are more likely to die from their injuries than women. 

Seniors should be aware of several things that can affect balance and lead to a fall. These include certain medications, inner ear problems, foot pain, weight changes and a vitamin D deficiency. Anti-hypertensive medications prescribed to keep blood pressure under control or decrease the risk of stroke and heart failure are all known to affect balance. A common side effect is orthostatic hypotension, a condition that happens if blood pressure gets too low; and results in lightheadedness, faintness and dizziness especially when the person attempts to stand from a sitting or lying down position. 

Medications that suppress the central nervous system may also result in a fall as they reduce alertness, and slow reactions and movements. Some of these types of drugs include anti-anxiety medications like diazepam (Valium) and lorazepam (Ativan). Additionally, antihistamines can cause drowsiness and are used in over-the-counter sleep aids like Nyquil and Sominex, or combined with acetaminophen in products like Tylenol PM.  

If you or a loved one takes medication to treat an overactive bladder or tricyclic antidepressants to help lessen chronic pain, they too can affect a person’s balance. Other drugs that increase the risk of a fall are opioids such as codeine, hydrocodone and oxycodone.

If you have arthritis, heart or lung issues, pain and weakness in the feet and legs, impairment due to a stroke, visual impairment, or other balance problems, a mobility aid can help you feel more independent, reduce pain, and increase confidence.  With a number of options available ranging from canes to walkers, wheelchairs, and scooters, you may be wondering which one is best for your needs. 

Experts explain that your choice of aids will depend on your disability and/or medical condition.  Canes are the most common choice and are good for someone with balance problems.  In fact, the journal Medical News Today, says that one in 10 adults over the age of 65 uses a cane.   However, even here there are decisions to be made.  While white canes are reserved for the visually impaired or blind, you may find a four-footed cane with its wider base offers more stability.  If you need to shift weight from the wrist to the arm, try a forearm cane.   

Walkers, also called Zimmer frames, provide stability and are used by an estimated 4.6 percent of people in the country over the age of 65.  Along with the basic walker, styles include rollators that have four wheels, handlebars and a seat.  Knee walkers are similar to a rollator but have a padded cushion to rest a knee on while the stronger leg moves it forward.  Walker-cane hybrids are another option that offers better support than a standard cane.

As We Age, Good Nutritional Habits are More Important Than Ever

The government first published nutritional recommendations in 1941, which became known as the Recommended Dietary Allowance (RDAs). At the time, recommendations were more concerned on ensuring the population “as a whole was adequately fed and free from scurvy, rickets and other wartime diseases of malnutrition.” These early guidelines did not address long-term issues such as heart disease and diabetes.

Since then, we have learned we need to focus on maintaining good nutritional habits well into our senior years to avoid or mitigate our risk for chronic diseases. One example is type 2 diabetes, which occurs when the body’s muscle, fat and liver cells stop responding well to insulin resulting in consistently high blood sugar. Doctors say type 2 diabetes can be caused by too much sugar and refined-carbohydrates in our diet, and to lessen our risk, we should maintain a healthy weight and minimize blood-sugar spikes as well as add more fiber to our diet.

Another thing an older adult should be aware of is muscle loss. Along with resistance training, weight-lifting, Pilates, and yoga, nutritionists say to maintain strength we need to include protein in our diet. Christine Ritchie, director of research for the division of palliative care and geriatric medicine at Massachusetts General Hospital and a professor of medicine at Harvard Medical School, says older adults’ diet should include more protein than when they were younger. A man in his 50s needs “about 25 to 30 grams at each meal (like a medium chicken breast or burger, a 5-ounce can of tuna mixed with mayo, 2 cups of cooked rice and beans, or a cup of low-fat cottage cheese).”

Government dietary guidelines report that we burn approximately 200 fewer daily calories after age 50, so if you keep eating like you did in your thirties or forties, you will probably gain weight. Also, as bone loss starts sometime in our early thirties and continues into old age, seniors should consume foods with calcium and vitamin D. The American Academy of Orthopaedic Surgeons recommends 1,200 milligrams a day of calcium for women and 1,000 mg for men. Additionally, eat fruits and vegetables in a bright range of colors for heart-protecting antioxidants.

While some people seem to not turn gray until an advanced age, most of us see our first gray hair sometime in our thirties. Experts say in time, everyone’s hair will turn gray, and with each decade after age 30, your chance of going gray increases by 10 to 20 percent. For those blessed with a very long lifespan, their hair will most-likely go completely white.

The process is gradual as each strand of hair a follicle produces remains the same color it starts out as. For instance a strand of blond hair stays blonde regardless of how long it grows (we typically keep a strand for up to 3 ½ years). Gray hair and eventually white hair is caused as our follicles produce less and less color as we age.

However, not all gray hair is a result of “age-related graying.” In some cases, experts say stress may play a role in how quickly someone grays. They point out that most likely it is not the stress itself that makes hair go gray but rather it triggers “a common condition called telogen effluvium, which causes hair to shed about three times faster than normal.” So if you are the right age, many of the new hairs that grow in may be gray.

When very young people start to turn gray, it can be a cause for concern. Premature hair graying (PHG) can be from a vitamin B12deficiency, thyroid disease, or smoking as well as vitiligo, which causes melanocytes (cells at the base of hair follicles that produce the hair’s color) to be lost or destroyed. Still researchers say turning gray is mostly influenced by the genes we inherit from our parents and we can get a better idea of when we will turn gray by looking at past generations.

Passing along Family Traditions

Do you celebrate Christmas each year with a large family get-together on Christmas Eve, host an annual family reunion with a barbeque and baseball game, or take yearly family vacations to new places? Most families have traditions that are unique to them, and experts say this is important as they help us feel connected and teach values. Traditions are rituals that can be big or small but are done with a sense of purpose and an awareness that something special is going on.

While forming the bonds of family togetherness can sometimes be hard to do in today’s busy world, the rewards are great. Family rituals provide a sense of home, security and identity that kids and the older generation need. They shape our holidays and birthday celebrations, bedtime routines, meals and more. As family expert, Meg Cox defines in her newly updated guide, The Book of New Family Traditions; family rituals are “any activity you purposefully repeat together as a family that includes heightened attentiveness and something extra that lifts it above the ordinary ruts.”

If you wish to create new traditions for your family, start by thinking about the things you did that shaped your own childhood or that of your parents, and try to apply it to your own family’s rituals and celebrations. They can be things we do frequently or just at special times; but if they have value, they may still be around for future generations.

The holidays are easy times to create family-oriented traditions and memories. At Christmas, have an annual ornament-making party, bake batches of Christmas cookies, or make and send out Christmas cards. Another idea is to bury a time capsule and have family members write a note and/or put in something that characterizes their interest at the time.

If your family loves playing games, set up a weekly or monthly game night. At the same time, you can make it extra yummy by having everyone make their own sundaes, or cook up a big batch of popcorn or nachos. In the fall, organize a trip to a local orchard to pick apples and then bring them home and have everyone help make pies or sauces. Instead of buying Halloween pumpkins at the supermarket, make a day of it and take the family to a pumpkin patch.

Remember to honor your family’s heritage by including traditional foods in your celebrations. And don’t forget about including religious and cultural traditions that shape our values, our memories and the very people we become.

Organizing Your Medical Records is Important - We Have Tips to Help

What would happen if you or a loved one had a medical emergency that required immediate attention? Could you or another family member relay the person’s medical history to hospital personnel as well as provide an up-to-date record of their medications? A study found that senior cardiac patients “who kept a personal health record enjoyed better health—perhaps because they and their caregivers could better see their health picture.” For suggestions on how to efficiently organize yours or a loved one’s medical records, take a look at some of the following tips experts share.

According to geriatric medicine physician Alicia Arbaje, M.D., M.P.H., the first step is to create a doctor directory for each family member. Include all of the various doctors they see as well as contact info and the role they play in the care team.

The next step is to put together the person’s medical history and current health information categorically and chronologically. Medical history should include pertinent health info on parents, siblings and grandparents. While medical records over a year old can be filed away, the current file needs to include information on past medical treatments, surgeries, accidents and hospitalizations. In addition the file should have a record of the current year’s doctor visits and hospital discharge summaries as well as pharmacy printouts of prescribed medications. Don’t forget to list any over-the-counter medications taken.

Another thing to keep in the file is a copy of the person’s living will and medical power of attorney. You should also give a copy of the person’s advance directives to their primary healthcare provider.

To save work, take advantage of the doctors’ electronic patient charts to access test results, doctor notes and other pertinent medical details. Lastly remember your medical records are only as good as the information so keep it up-to-date.

Steps to Improve Your Cognitive Functioning

Researchers says that even if we are not developing Alzheimer’s or other forms of dementia, as we grow older, our memory, decision-making, and ability to learn simply begin to wear down.   This process is referred to as “cognitive aging” and doctors now say there are evidence-based steps we can take to improve our cognitive functioning.  

The published study from the Institute of Medicine titled “Cognitive Aging: Progress in Understanding and Opportunities for Action,” detailed three ways we can protect and bolster our aging brain.  The study’s authors are Dr. Dan Blazer, chair of the report and professor emeritus of psychiatry at Duke University Medical Center, and Dr. Valentin Fuster, director of Mount Sinai Heart and physician-in-chief at the Mount Sinai Hospital.  The report is intended to provide relevant information to both physicians and patients. 

According to the study, the first thing we can do to help keep our brain healthy (and other parts of our body) is to engage in light exercise every day.  Activities such as walking, gardening, swimming or biking help increase the blood flow to the brain, which helps to maintain and improve its functioning.  

The Cognitive Aging report also found that in order to reduce our risk for or to better manage cardiovascular disease, we need to eliminate unhealthy habits such as smoking and maintain a healthy weight and blood pressure.  It recommends that if you do have high cholesterol, you follow your doctor’s instructions and continue to have the necessary blood tests done.  

The third key to improving and maintaining good cognitive functioning is to consult with your doctor to review the list of medications you are currently taking.  Dr. Blazer explains that, “Older adults are sometimes on more medications than they need to be on."  He adds, “Many, if not all of them, can potentially affect your cognitive functioning.”

Enjoy the Beautiful Weather but Take Care of Yourself!

Now that we are moving into the lazy days of summer, many of us cannot wait to get out and enjoy the warmer weather.  However seniors should know that higher temperatures and humidity can be dangerous for them if they are not careful.  According to a study from the University of Chicago Medical Center, 40% of heat-related fatalities in the U.S. were in people over 65.  Signs of heat-related illness include headache, confusion, dizziness, or nausea; and if you do get overheated, it is important to get relief quickly. 

To prevent hyperthermia (overheating), the National Institute of Health (NIH) cautions the elderly to avoid the outdoors during the hottest part of the day, and warn it is not only 100°F temperatures that put them at risk.  They explain seniors are more susceptible to heat-related illness for a number of reasons including the normal aging of sweat glands and changes to the skin.  

Also medications used to treat health conditions common in the elderly such as “diuretics, sedatives, tranquilizers, and some heart and high blood pressure medicines;” can make it harder for the body to cool itself.  In addition heart, lung or kidney diseases can cause heat-increasing fevers. 

The American Academy of Dermatology suggests when seniors do go outside to do it in the early morning or evening and sit in the shade.  They also recommend using sunscreen of SPF 30 on any exposed skin every day, whether you are inside or out.  Additionally drink plenty of water and fruit or vegetable juices, and avoid caffeinated or alcoholic drinks.  If you don’t have air conditioning, keep the blinds closed during the heat of the day; and take cool showers, baths or sponge baths if you feel overheated.  Doctors also suggest getting to a place where you can sit in air conditioning like the mall, and if you cannot cool down; seek medical help as heat exhaustion can progress to heat stroke.

Another all-too common problem for seniors is dehydration.  As we age, the thirst sensation diminishes, and instead of our bodies signaling the need for fluids, we don’t feel thirsty.  Dehydration is a common cause of hospitalization in the elderly, and as early signs include dry mouth, fatigue, dizziness and muscle cramps, they are often attributed to other problems.  Anne Vanderbilt, CNS, a geriatric nurse says, “By the time they are thirsty, that’s already an indication of early dehydration,” so have water available to sip throughout the day.   Enjoy the nice weather but please play it safe!
