If you or a loved one is starting to feel less steady on your feet and at risk of falling, a walker, cane or even a wheelchair or scooter is probably advisable. However, before you start using a hand-me-down walker or buy any-old cane; experts warn that one size does not fit all. In the case of a walker, to be an effective and safe mobility aid, it is important to have an expert help you pick out the best type and size for your needs.

As AgingCure.com points out, “The wrong device can negatively affect a senior’s stability, cause posture-related problems and even result in pain. Mobility aids come in all shapes and sizes, so selecting the right equipment and achieving a proper fit usually requires a professional’s assistance and expertise.” To get started determining what your needs are, experts recommend seeing your healthcare provider to discuss any problems you are having with your activities of daily living (ADLs). If they recognize a need for a mobility aid, they may refer you to an occupational or physical therapist.

The following are a couple of basic guidelines to know before you get started. A cane can help provide balance and stability and is ideal for someone who needs to support no more than 25% of their body weight. It is most useful for those minimally impaired or with weakness in one leg. Walkers are available in a variety of models but regardless of the style a “correct height of a walker is measured from the floor to the wrist, and the elbows should be flexed in a range of 15 to 20 degrees.” Experts also say the walker must be large enough to provide front and side support for the user.

A motorized scooter may be the answer for someone who has lost the ability to walk any distance but still has an adequate amount of strength in their arms and upper body to stay upright as well as drive the scooter. Again there are a variety of models including indoor and outdoor ones as well. Keep in mind though that most insurance companies and Medicare Part B typically only cover scooters for in-house use.

Meaningful Activities Help Those with Dementia

It is important for family members and caregivers to remember that while a person with Alzheimer’s or another dementia-related disease may be declining cognitively, they still have some abilities. Encouraging them to stay involved in meaningful activities is a good way to give them a sense of self-worth and purpose.  In fact, a published report from the National Institute of Health states, “engaging older persons with dementia in appropriate activities has been shown to yield beneficial effects such as increasing positive emotions, improving activities of daily living (ADL) and improving the quality of life.” It goes on to say it is important for those with dementia to stay as active and engaged as possible and that “it may even help to maintain quality of life in the more advanced stages of the disease.”  

As to what kinds of activities are the most effective, studies suggest trying to stimulate all five senses: sight, touch, sound, taste and smell; and the creative arts are a good place to start.  Singing is not only enjoyable but several studies have shown it to be one of the last abilities many of those with Alzheimer’s lose.  People can often remember the words to a song long after many other memories are gone.  Listening to music and dancing are other activities beneficial to those with dementia.  Dancing has even been shown to help stimulate social interaction, enhance mood, reduce anxiety and depression, and increase self-awareness and self-expression.  Walking and getting outside can also lift spirits, but as gait can be adversely affected by Alzheimer’s, evaluate the person’s ability to walk safely.

What is an Occupational Therapist?

As we age, we can experience health issues such as a stroke or other disabilities that may benefit from the services of an occupational therapist (OTs).  When asked, many people have no idea what an OT does; while others think they help individuals find work.  Actually, an OT may be an instrumental part of your healthcare team who will work with you to facilitate your best recovery.  If you are wondering what the difference is between a physical therapist (PT) and an OT, a PT focuses on restoring or improving movement, strength, and range of motion; while an OT works on improving the motor skills you need to perform daily tasks. 

If a loved one has suffered from a stroke, they may have lost the ability to perform tasks known as Activities of Daily Living (ADLs), which include bathing, toileting, dressing, eating, grooming, etc.  The job of the OT is to teach the person how to carry out daily, functional activities despite the impairment in order to improve quality of life and maximize independence. 

For instance, if the stroke caused damage to one side of the body, an OT focuses on therapies that promote motor recovery.  One common intervention is to limit or constrain the person’s movement on the unaffected arm, while having them use the affected arm to do repetitive or functional tasks.  OTs may also recommend special equipment to improve their patients’ lives such as a wheelchair, walker, electric can opener or toothbrush, a special keyboard for the computer, or eating aids and then teach the person to use them.

What is Mild Cognitive Impairment (MCI)?

If you or a loved one is experiencing some mild memory problems or were recently diagnosed with Mild Cognitive Impairment (MCI), you may find yourself wondering what that means for the future. MCI is defined as the fine line between what we think of as normal memory or thinking and dementia. Dr. Scott M. McGinnis, an associate neurologist at Brigham and Women's Hospital in Boston, clarifies it by explaining, “We diagnose dementia when a person has changes in memory and thinking that significantly interfere with functioning in usual activities.” He cites examples such as getting lost driving to familiar places or having difficulty writing a check.

MCI is diagnosed when a person has changes in their memory and thinking that goes beyond normal aging but does not qualify as full-blown dementia. Medical experts explain that MCI may be a precursor to Alzheimer’s disease but are also quick to add that not all cases of MCI result in Alzheimer’s but rather may increase the person’s risk. In fact, MCI is caused by other things including medications and infections.

Doctors suggest one way to cope with MCI is to get into the habit of writing out a list of “to do’s” each day and checking it throughout the day. Some people want to know more and thus pursue medical tests that can identify if they will develop Alzheimer’s eventually, while others wish to take a more wait and see approach. Whatever your decision, medical professionals say it may be helpful to learn as much as you can about MCI as a way to help you cope.

Studies Show Healthy Habits Reduce the Risk of Alzheimer's Disease

While there is no cure or medical treatment to prevent Alzheimer’s disease there is some encouraging news on how we can slow down the risk of the disease in our own life, even if we have a higher genetic risk.  Researchers say that several studies indicate that by following a combination of healthy lifestyle habits we lower our risk of cognitive decline, and the more of them we adopt, the better it is for us.  

The main health habits linked to slowing down the onset of Alzheimer’s include regular exercise, cognitive stimulation, a brain-healthy diet, light to moderate alcohol consumption and not smoking.  In fact in one study, researchers from Rush University in Chicago found that participants who “followed four out of five lifestyle behaviors over a six-year period had a 60 percent lower risk of developing Alzheimer's dementia when compared to people who practiced only one or none of these habits.” 

Another study from the University of California San Francisco reports that people who smoked had twice the risk of developing cognitive impairment as compared to non-smokers or to someone who had kicked the habit.  Also, a study out of the UK found that even those with an increased risk for cognitive decline who followed a healthier lifestyle had a 32% lower risk.

Do you celebrate Christmas each year with a large family get-together on Christmas Eve, host an annual family reunion with a barbeque and baseball game, or take yearly family vacations to new places? Most families have traditions that are unique to them, and experts say this is important as they help us feel connected and teach values. Traditions are rituals that can be big or small but are done with a sense of purpose and an awareness that something special is going on.

While forming the bonds of family togetherness can sometimes be hard to do in today’s busy world, the rewards are great. Family rituals provide a sense of home, security and identity that kids and the older generation need. They shape our holidays and birthday celebrations, bedtime routines, meals and more. As family expert, Meg Cox defines in her newly updated guide, The Book of New Family Traditions; family rituals are “any activity you purposefully repeat together as a family that includes heightened attentiveness and something extra that lifts it above the ordinary ruts.”

If you wish to create new traditions for your family, start by thinking about the things you did that shaped your own childhood or that of your parents, and try to apply it to your own family’s rituals and celebrations. They can be things we do frequently or just at special times; but if they have value, they may still be around for future generations.

The holidays are easy times to create family-oriented traditions and memories. At Christmas, have an annual ornament-making party, bake batches of Christmas cookies, or make and send out Christmas cards. Another idea is to bury a time capsule and have family members write a note and/or put in something that characterizes their interest at the time.

If your family loves playing games, set up a weekly or monthly game night. At the same time, you can make it extra yummy by having everyone make their own sundaes, or cook up a big batch of popcorn or nachos. In the fall, organize a trip to a local orchard to pick apples and then bring them home and have everyone help make pies or sauces. Instead of buying Halloween pumpkins at the supermarket, make a day of it and take the family to a pumpkin patch.

Remember to honor your family’s heritage by including traditional foods in your celebrations. And don’t forget about including religious and cultural traditions that shape our values, our memories and the very people we become.
