Why it is Important to See a Doctor if You are Worried About Memory Loss

The Alzheimer’s Association recommends that anyone worried about memory loss to make an appointment with their health care provider.  While we all know it is scary just thinking about the possibility of having Alzheimer’s or another dementia-related disease, the Alzheimer’s Association has a list of reasons why it is to your advantage to know.  

The process for getting a diagnosis starts with a full evaluation with a healthcare provider that includes your medical history and any medications you are taking as well as a series of mental status tests.  If the initial evaluation indicates problems, your doctor will recommend further physical and neurological exams, diagnostic tests and brain imaging.

One of the top reasons to be evaluated for Alzheimer’s is your symptoms may be from another treatable cause.  If it is determined you have Alzheimer’s, experts explain while there is no cure for the disease, there are treatment options that can lessen the symptoms including memory loss and confusion for a limited time.  Also, they say an early diagnosis gives you a better chance of benefiting from treatment.

An early diagnosis also means you may be eligible for clinical trials that could provide some medical benefit.  In addition, recent research suggests that certain lifestyle changes such as “controlling blood pressure, stopping smoking, participating in exercise, and staying mentally and socially active, may help preserve cognitive function.”   

Perhaps most importantly you will have more time to plan for your future and to do things now, rather than wait.  Planning for the future also helps family members know what your wishes are and prevents future feelings of guilt or arguments over your care.  The Alzheimer’s Association adds that an early diagnosis can save your family and the U.S. Government money spent on care.

Nearly Half of All Seniors Over 75 have Difficulty Hearing

The National Institute of Health reports approximately one in three people between the ages of 65 and 74 has hearing loss and nearly half of those older than 75 have difficulty hearing. Hearing loss causes someone a range of problems including difficulty understanding and following a doctor’s advice, responding to safety warnings, and hearing doorbells, phone calls, and alarms. Another all-too common problem is it can be hard to follow conversations with family and friends, and many start avoiding situations where they may be frustrated or embarrassed. 

Medical experts advise asking your primary care physician for a referral to an otolaryngologist, an audiologist or a hearing aid specialist. An otolaryngologist is a doctor who specializes in diagnosing and treating diseases of the ear, nose, and throat. They will try to determine the cause of your hearing loss and treatment options. They may refer you to an audiologist who is trained in measuring the degree of loss as well as recommend treatment, which may include fitting you for a hearing aid. There are three different types of hearing aids and it may take some trial and error to find one that works for you (most can be purchased for a trial period).  

In some cases, doctors may recommend Cochlear implants, small electronic devices surgically implanted in the inner ear that help provide a sense of sound to people who are profoundly deaf or hard-of-hearing.  Other treatment options include hearing assistive devices for your phones to amplify sound; closed circuit systems for churches or theaters; and lip or speech reading classes.

Also, remember it may not be dementia as there are medical conditions, reactions to medications, and nutritional and hormonal issues that can cause dementia-like symptoms.  This is why you need to sit down and have a talk with you parent about the problems they are having and the need to make an appointment with their healthcare provider for a full medical exam.   

Experts say when you are ready to have the “conversation,” be honest and try to keep your voice calm.  Pick a time and place where you will not be interrupted and be sure to include in your homework different care options in case they ask.  Also, be ready to talk less and listen more as they may have also been worried about some of the changes they have been experiencing.  However, keep in mind it is common for a person to be confused, in denial, and/or withdraw.

Help Reduce Your Risk of Falling with Balance Exercises

According to the National Institute of Health (NIH), every year more than a third of people over the age of 65 suffer from a fall, and many of those accidents require a trip to the emergency department.  In addition to serious injuries, some falls even result in death.  To help mitigate the risk, experts recommend that along with safety modifications to the home that elderly adults also do balance exercises.

Balance exercises are relatively easy-to-do and can improve both your balance as well as increase lower body strength.  Ask your doctor for some suggestions or take a look at the Internet. Most do not require special equipment other than someone to assist you if needed.  The exercises can be done throughout the day and each one only takes a few minutes to do.  

One example of a balance exercise is called Tightrope Walk.  This easy exercise improves balance, posture, and core strength.  Start in a standing position and lift your arms extending them out to your sides.  Now walk in a straight line keeping your gaze focused on a fixed point in the distance.  Each time you raise your foot, pause with it in the raised position for 2 to 3 seconds.  Alternating your feet, continue for 20 to 30 steps.   

Another exercise is Rock the Boat, which starts by standing with your feet hip-distance apart and extending your arms out to your sides.  Lift your left foot off the floor and bend your knee to bring your heel toward your bottom holding it there for up to 30 seconds.  Now do the same with the other side.  Do each side 3 times.

Help Maintain and Enhance Your Memory

In an article in the US News & World Report, experts say forgetfulness is not an inevitable part of growing older and there are things we can do to help maintain and enhance our memory function and overall brain health.  Furthermore, they also benefit whole body health.      

One of the first things they recommend is to never stop making “physical activity a priority as it optimizes blood flow to the entire body, including the brain.  An adequate supply of blood and oxygen to the brain helps to keep our memories sharp.”  Researchers explain that the type of exercise is also important and recommend a minimum of 30 minutes a day of cardio exercise such as running, power walking, biking, or swimming.  However, before beginning any new exercise regime, be sure to check with your healthcare provider.  

In addition, continuing to learn new things can improve our memory as studies have found a correlation between challenging our brain and maintaining brain health.  This could include activities such as playing an instrument, starting a new hobby, learning a language, or just broadening your reading habits.  

Other advice that not only enhances cognitive thinking but overall health includes getting enough sleep each night.  Research has shown that “powering down on a nightly basis allows the brain to heal and restore itself, clearing toxins that can lead to Alzheimer’s disease and other dementia types.”

Are You a Senior Who Wants to Start Exercising Again?

If you are a senior who wants to start exercising again, you may not know how to begin.  For one thing, you probably realize you are not as fit as you used to be, and are not up to the same degree of physical activity that you once did.  And of course, before you begin any new form of exercise, you will need to check with your doctor to make sure you are fit enough, especially if you have osteoporosis, an injury, or balance issues.  

Once you get the go ahead from your healthcare provider, stretching is a good place to begin.  In fact, studies show that as we grow older our flexibility decreases by up to 50 percent in some joints.  Another good thing about stretching is many exercises are appropriate for any level of fitness including ones that can be done sitting in a chair.  Stretching can help you maintain your flexibility and mobility as well as loosen up some of your stiffness in your back, neck and shoulder muscles.

Another plus about stretching exercises is that most of them do not require special equipment and can be done anywhere.  The Silver Sneakers website recommends that as you stretch, “to breathe deeply, and go slowly. Listen to your body, and never force a movement that causes pain.”  They remind seniors it is okay if they cannot bend very far as over time they will loosen up some.  Safety is always important and you do not want to hurt yourself.  Other exercises especially good for seniors include chair yoga, resistance band workouts, walking, and dumbbell strength training.

As We Age, the Risk of Falling is a Serious Concern

The Mayo Clinic reminds older adults it is important they take proper precautions to prevent falls.  Medical conditions, the resulting medications needed, and a more sedentary lifestyle can result in weaker muscles and balance problems that can lead to a fall.  Also please keep in mind, a fall for an older adult can cause injuries that are very serious such as a broken hip, head trauma, or in the most severe cases, death.   

However, the solution to preventing falls is not to stop moving.  The medical experts at the Mayo Clinic explain being inactive is bad for our health regardless of how old we are and it is vital that we keep moving.  To that end, they suggest a commonsense six-part plan to keep seniors safely mobile.    

Their first recommendation is to set up an appointment with your doctor.  Bring to the visit a list of all your medications, and be prepared to discuss with your doctor any side effects of the drugs that may put you more at risk for a fall such as anti-depressants or sedatives.  It may be possible to eliminate or substitute the medication for something else.  Also, talk to your doctor about whether you have fallen and if it was due to your medical condition.   

In addition, discuss with your healthcare provider what type of exercise would be best for you.  Typically walking, swimming and something like tai chi are gentle exercises that are safe for most; and offer the benefit of strengthening muscles, improving balance, and gait.  

Other ways to lessen your risk for a fall begins by reviewing your home with an eye to fall prevention.  Start by checking hallways and stairways for adequate lighting, and add more if needed.  Secure loose rugs and make sure you have wide clear paths for walking around your home.  Wear proper fitting shoes with nonskid soles and use assistive aids like a cane or walker if needed to help maintain your balance.  Install grab bars and railings where needed.

Should You Still Wear a Mask After Vaccination?

Now that many seniors are receiving their COVID-19 vaccines, we are all getting impatient to get back to normal life.  However, the Centers for Disease Control and Prevention (CDC) cautions that even after we receive our second dose of the vaccine, we still need to wear our face mask, at least for a while.  

To start with, it will take some time to build up our immunity to the virus as well as the fact that the vaccine is not 100 percent effective against COVID-19.  Pfizer-BioNTech reports that “protection doesn't start until 12 days after the first shot and that it reaches 52% effectiveness a few weeks later.”  Their studies have shown that around a week after you receive your second vaccination, the effectiveness rate is 95%.  With the Moderna vaccine, an individual is reported to have a “protection rate of 51% two weeks after the first immunization and 94% two weeks after the second dose.”

Another reason the CDC recommends continuing to mask up around people outside your “pod,” is that doctors do not know if vaccinated people who may still get a mild or asymptomatic case of COVID-19 can spread the virus to others.  As more and more people get vaccinated, they hope to have more answers to those questions.  For now to protect yourself and others, wearing a mask when in groups, socially distancing at least 6 ft. apart, and frequently washing our hands, are all good advice to follow over the next several months as healthcare providers work to get more people vaccinated.

During the coronavirus outbreak, however, changes have been made in dining protocols to protect both residents and team members. We are happy to report that COVID-19 vaccinations began at our community at the beginning of January. In addition, much of our food is modified out of respect for some of our residents’ compromised eating abilities and include finger foods, texture-modified diets and easy-to-recognize balanced meals that taste as good as they look.   

As dementia care specialists, we focus on nurturing and supporting the essence of the individual, regardless of the level of memory loss.  In order to create an effective individualized care plan to guide our daily interactions and communication techniques, we meet with the senior and their family to learn as much as we can about the person’s background, talents and preferences even before they join our community.  Our specialized care incorporates music, art, dance, and aromatherapy to heal and comfort residents while taking into account the uniqueness of each person.

Meaningful Activities Help Those with Dementia

It is important for family members and caregivers to remember that while a person with Alzheimer’s or another dementia-related disease may be declining cognitively, they still have some abilities. Encouraging them to stay involved in meaningful activities is a good way to give them a sense of self-worth and purpose.  In fact, a published report from the National Institute of Health states, “engaging older persons with dementia in appropriate activities has been shown to yield beneficial effects such as increasing positive emotions, improving activities of daily living (ADL) and improving the quality of life.” It goes on to say it is important for those with dementia to stay as active and engaged as possible and that “it may even help to maintain quality of life in the more advanced stages of the disease.”  

As to what kinds of activities are the most effective, studies suggest trying to stimulate all five senses: sight, touch, sound, taste and smell; and the creative arts are a good place to start.  Singing is not only enjoyable but several studies have shown it to be one of the last abilities many of those with Alzheimer’s lose.  People can often remember the words to a song long after many other memories are gone.  Listening to music and dancing are other activities beneficial to those with dementia.  Dancing has even been shown to help stimulate social interaction, enhance mood, reduce anxiety and depression, and increase self-awareness and self-expression.  Walking and getting outside can also lift spirits, but as gait can be adversely affected by Alzheimer’s, evaluate the person’s ability to walk safely.
