OTC Drugs Pose Risks for Seniors

All too often people think of over-the-counter drugs (OTC) as being safe and not having the side-effects or drug interactions their prescription drugs might have. Experts say this attitude could put you at risk as many OTC drugs are not as harmless as we think, especially for older adults. AARP cited a statistic showing as many as 178,000 hospitalizations a year are caused by a reaction to OCT medications and adults 65 years and older were more commonly harmed.  

One reason older adults are more vulnerable to adverse outcomes are many already take several prescription drugs. A report from the Lown Institute, a non-profit organization, reported nearly half of adults 65 years and older take five (known as polypharmacy) or more prescription drugs daily. Another 20 percent over 65 take 10 or more daily. 

This increases the risk of interactions when taking OTC medication for pain relief, stomach issues, allergies or other needs. In addition, the AARP article explains as we age, our bodies lose muscle and gain fat, which slows down the process of getting rid of medications. One of the organs responsible for processing medication is the kidneys, which start to work less effectively to filter and eliminate waste causing medications to build up. Aging livers also work less effectively to break down medications. 

Common OTCs like acetaminophen (generic name for Tylenol) can cause damage to the liver. Ann M. Hester, M.D., a board-certified internal medicine physician based in Highland, Maryland explains overuse of acetaminophens is the leading cause of deadly liver failure. Seniors on blood thinners should be aware that ibuprofen (Motrin and Advil) and naproxen (Aleve) OTCs drugs may result in stomach bleeding and ulcers. Moreover, doctors no longer recommend people take a daily aspirin as a preventive measure toward heart disease as it too may cause stomach bleeding. 

People with high blood pressure should be aware that decongestants containing phenylephrine or pseudoephedrine like Sudafed can increase blood pressure. It can also cause problems to combine an OTC pain reliever with a decongestant as it too raises blood pressure putting the person at risk for a stroke; and men with prostrate issues are at risk for urinary problems. There are other OTCs that seniors should be wary of so before you take one, be sure to check with your healthcare provider.

Signs it’s Time for Cataract Surgery

Statistics show cataracts affect approximately 24 million Americans and is the leading cause of blindness throughout the world. They occur when the eye’s natural lens becomes cloudy due to the breakdown of its proteins, resulting in blurry, hazy or less colorful vision. Age is the primary reason for cataracts and typically starts around the age of 60. As they take a long time to develop; many people do not notice the gradual blurring of their vision. Individuals with diabetes may experience the problem at a much younger age. 

If during an eye exam your doctor mentions you have the start of cataracts, they may tell you it is not to the point where surgery is required; but may recommend increasing the strength of your prescription eyeglasses. If you are told the problem is far enough along to warrant surgery keep in mind it is a routine procedure.  However, you should talk to your doctor about the benefits and risks to determine if it is the right time for you. 

There are a few signs that may make it easier to decide if you want to do the surgery. These include having a hard time driving at night and not being able to read or watch TV because of the cataracts. Double-vision and seeing colors as faded or yellowish are other signs. Another reason it may be time to schedule the surgery is the cataracts are making it difficult for your doctor to do a thorough exam of your eyes. 

One of the best ways to slow down cataracts is to wear sunglasses with UV protection and to quit smoking. If you see “halos” or can’t handle glare while driving at night, stop driving after dark. Experts say cataract surgery has a high success rate but it can take anywhere from two to six weeks to fully recover.

Comforting a Friend Who is Grieving

Many of us have watched a good friend go through the grieving process after losing a loved one and have been frustrated by not being able to do more to comfort them. Experts explain this is a common feeling and provide some suggestions to help.

One of the first things to remember is everyone experiences grief in their own way. Heather L. McGinty, assistant professor in the department of psychiatry and behavioral health at the Ohio State University Wexner Medical Center in Columbus, explains grief triggers a range of emotions, sensations and behaviors and can be complex. 

A person’s emotions may swing from tearfulness to anger or denial, and as a friend, it can be hard to know what to say. Loneliness, betrayal, fear and acceptance are some of the other emotions grief brings up. Some people even experience physical symptoms such as headaches or stomachaches. 

If you want to help, grief counselors say to first make sure the person wants it; and recommend asking if they want the type of support you hope to offer, such as spending time with them. Some people are very private and prefer to grieve in their own way. The most important thing is to be supportive of their wishes and be prepared to listen rather than hand out advice. Also, be specific like letting them know you want to take them to lunch when they are ready, or bring over a meal, run errands or do some yardwork. 

Experts say avoid telling the person you know how they feel. Again, everyone experiences grief differently. Even if you don’t understand the way the person is acting out their grief, be supportive not judgmental. Lastly, don’t say things like “everything happens for a reason” or “they are in a better place.” If you think they would benefit from professional support, be sure to encourage it.

Learn More About Pacemakers

According to the National Heart, Lung and Blood Institute, the most common reason for a pacemaker is when the heart beats too slowly (bradycardia), or it pauses, causing fainting spells or other symptoms. The device also prevents or treats a too fast or irregular heartbeat (tachycardia). For patients with heart failure, a pacemaker is used to help the heart chambers beat in sync, and enable the heart to pump blood more efficiently.

Pacemakers use low-energy, electrical pulses to control the rate and rhythm of the heartbeat, and the type of pacemaker you get depends on your situation. A traditional pacemaker sends electrical pulses through wires (or leads). It is placed in the chest or abdomen; and wires are connected to electrodes inside one to three heart chambers. This type of pacemaker includes single- and double-lead pacemakers, or biventricular pacemakers also known as cardiac resynchronization therapy (CRT) devices.

A newer version of the pacemaker is a wireless pacemaker without wires, which is about the size of a large pill capsule. It is placed inside a chamber of your heart through a small tube inserted in one of your veins and doesn’t require surgery. The device sends pulses to the right ventricle. Typically the battery life of a pacemaker is between 8 and 13 years.

After the procedure, most patients spend a night in the hospital so doctors can make sure the device is staying in place, doing its job, and programmed correctly. You will be given a card with information regarding the device’s settings and the hospital where you got it, which needs to be always carried with you.  

Once home or in assisted living, restrictions include not lifting heavy items or putting your arms over your head to avoid shifting the device or the wires. Your medical team will schedule a check-up over the next few weeks. 

Living with a pacemaker includes avoiding close contact (at least 6” away) with devices having strong magnetic fields as they can disrupt your pacemaker. When using your cell phone, put it on speaker phone or hold it to the ear on the other side of your body from your device. Don’t put it in your shirt pocket. Household items like microwaves, electric blankets, and heating pads are usually safe if they are working properly. The risks of airport screening are low, but show your pacemaker card to be individually screened. Contact your doctor with questions and problems, and if you think your pacemaker has stopped working, contact them immediately.

The Search for an Assisted Living Community

If you have started searching for assisted living for a parent, you know it can be overwhelming.  You are probably looking for a place that fits their budget, care needs, and lifestyle preferences.  Moreover, you know it is not a decision to make lightly as you will not want to move mom or dad again after just a few months.  That is why the experts suggest that if the circumstances allow you should take your time.  

The Internet has several checklists and suggestions on what to look for when searching for assisted living communities including questions to ask when talking to the staff.  One thing everyone agrees on is that it is a good idea to take a tour of the community.  

Questions you will want to ask include suite sizes and configurations, pricing information and availability.  If you plan to have both of your parents move in, ask if they can share a suite.  Also, if applicable, find out if the community supports Companion Living (where two people of the same sex share a suite).  

You will also want to learn more about the care services your loved one will receive. How is the level of care determined?  Will they have access to care 24/7?  How close is the community to medical care facilities and how are medical emergencies handled? Are nurses available and if so, how often?  What is the staff-to-resident ratio and do the staff receive special training?  Do any of the caregivers have CPR training?  

In addition, ask about programs and activities available to residents.  If possible, get a calendar of events to review later.  Also, ask about the dining experience.  Are meals offered all day?  Is the dining room full service?  Can a resident have meals delivered to their suite?  If your parents have diet restrictions, find out if they can be accommodated. 

Location is another thing to consider as you will want a community that is convenient for family and friends to visit.  Be sure to ask if you will be allowed to visit at any time or if there is a set schedule when residents can receive visitors.

Way too often older adults have a fear about assisted living that is based on misinformation. An article in the US News & World Report finds moving mom or dad to an assisted living community may be a bargain compared to paying for extensive in-home care. With an average cost of $25 to $27 an hour, in-home care can add up fast. For example, if you want someone with mom or dad while you are at work nine hours a day for five days a week, you are already spending $4,500 or more a month. This still leaves your parents without support on the weekends, and doesn’t include any support for problems which may occur during the evening and overnight. 

An assisted living community includes room and board and housekeeping services as well as a variety of wellness programs and activities. It also has 24/7 staffing and suites with emergency call systems, handicapped-accessible bathrooms and other modifications.  

If a family feels assisted living would be the best place for a parent but the cost is holding them back, experts say to thoroughly research all available payment options. This can include long-term care insurance and some private health insurance; or social programs such those offered by Veterans Affairs or Social Security Disability Insurance. A San Francisco based executive VP for A Place for Mom, Sue Johansen, suggests families consult with a financial advisor or a trust attorney. 

Remember, once you find the assisted living community you want, ask about the difference in pricing regarding room size or floor plan, and/or location in the community (view, balcony, etc.). You will need to factor in annual increases so be sure to ask what those have been over the last couple of years. Also, does the community require a deposit, move-in fee or community fees; and what type of notice does your parent need to give to move out?

To get the most benefit from mental activities, the experts say it is best to vary the type of games we play. Shifting from games that rely on speed, rapid decision making or memory help strengthen and tap into multiple ways to engage the brain. Studies show it is important to keep challenging ourselves and pushing the bar toward more difficult activities. Another thing to keep in mind are the studies linking socialization to a decrease in dementia. Watch for Signs of Illness When Caring for a Person With Alzheimer’s Disease

Caring for a person with Alzheimer’s disease can be a time-consuming job. As the disease is progressive and symptoms worsen over time, the person’s needs also evolve. Things they used to do for themselves may no longer be possible or safe. Moreover, experts say like many older adults, a person with Alzheimer’s is likely to experience health changes over time. Therefore, it is important he/she continues to get routine checkups with their primary care physician as well as with any specialists they may be seeing.

The National Institute on Aging (NIH) says some of the problems caregivers should look for are signs of fever, or sickness like the flu or pneumonia. They say a fever could be signs of infection, dehydration caused by lack of fluids, heat stroke or even constipation. When taking a person’s temperature, use a digital thermometer rather than a glass one as the person might bite down on the glass and break it. Let their doctor know right away if they do have a fever. To help prevent serious illness from flu or pneumonia, have the person get a yearly flu shot and a pneumonia vaccine if recommended. 

Dehydration can occur in people with Alzheimer’s as they may forget to drink fluids throughout the day. Caregivers should look for signs of dry mouth, dizziness and rapid heart rate. Experts say dehydration can even cause hallucinations; however, they are also a symptom of Alzheimer’s disease. 

As the disease progresses, the person will most likely need help brushing their teeth. It is important to look for any sores in the mouth or oblivious signs of dental problems. 

NIH cautions caregivers to be on the lookout for signs of pain as the person may not have the ability to tell you. They recommend watching the person’s face for outward signs of pain, or sudden changes in their behavior such as yelling or striking out. If you are not sure what is happening, call their doctor for help.

Reduce Your Risk of Falls

Many of us have heard about or know a senior who has had a fall that led to a serious injury. Each year statistics report one out of every five falls causes an injury, such as broken bones or a head injury; and approximately 3 million older adults are treated in emergency departments for a fall injury. Falls resulted in over 38,000 senior deaths in 2021. 

The Harvard Medical School reports the highest risk factor for a fall is a previous fall. While a senior may have fallen or tripped and not thought much about it, as they only twisted an ankle or didn’t sustain a serious injury, the event may be a precursor to another fall. 

There are things seniors in assisted living can do to protect themselves from a fall and it starts by being aware of any risk factors you may have including vision and hearing problems, which can make a fall more likely. Experts say stiff joints can hamper your gait and balance and make it easier to fall, as do certain medications for blood pressure and antihistamines. As a deficiency in vitamin D decreases a person’s bone mineral density and strength of the bones, it too can cause instability and raise the risk for a fall. Experts report even something as unrelated as a head cold or the flu can cause dizziness and increase your risk.  

To help boost core strength and improve balance, talk to your healthcare provider about the best type of exercises to do. Other steps to decrease the likelihood of a fall are to keep moving to loosen stiff joints and help build strength, wear sensible shoes and remove home hazards. Finally make sure your home has plenty of good lighting.
