Osteoarthritis is More Common in Older Adults

The Lancet journal reports the most common form of arthritis is osteoarthritis (OA); and it is estimated to affect 500 million people worldwide (7% of the global population). In the US, over 32.5 million have OA, or 1 in 5 adults. Of Americans with OA, 70% are over the age of 55; and it is one of the most common causes of disability in adults.  

Females are more likely to have OA with many cases occurring after menopause. Research suggests the resulting physiological changes in a woman’s body accounts for the elevated number of cases. Obesity is another risk factor, and may be due to the increased stress on weight-bearing joints. Others are more at risk for OA due to their occupation such as agricultural work, cleaning, construction and carpentry.

OA develops as the cartilage covering the end of a joint bone begins to wear away; and the joint rubs against a rough surface rather than smooth cartilage. With repeated use of a joint and the changes in the tissue over time, the condition is more common in older adults. Pain and stiffness associated with OA can impact a person’s day-to-day life and lead to depression and anxiety as well as make it harder for them to control their weight.  

While exercise has been shown to reduce pain in people with OA, it also comes with challenges. Many fear making the pain worse, and instability can cause a person to feel like they may fall or contribute to a lack of range of motion and mobility. 

However, exercise can be very beneficial to people with OA. Julie Lombardo, a physical therapist and CEO of Capitol Physical Therapy in Madison, Wisconsin, says, “It’s easy for people who encounter hurdles to give up on exercising when that is the last thing they should do.” Some exercises are better than others for people with OA; and consult with your healthcare provider if you are new to exercise, have high blood pressure or heart disease, or are obese.

Recent studies show more women die from heart disease than from all forms of cancer. Perimenopause and post-menopausal women are at the most risk from heart disease, and the older they get, the higher the risk. Doctors say the drop in estrogen that occurs in women during and after menopause is why older women have a higher risk for cardiovascular disease. 

Robin Noble, MD, a board-certified OB/GYN, explains before menopause, estrogen has a vasodilatory effect on the vascular system. This anti-inflammatory effect fights against the development of the type of inflammation that causes cardiovascular plaques. Once a woman’s body experiences a drop in estrogen, it creates problems such as higher cholesterol levels, higher blood pressure and an increase in visceral fat – all linked to plaque buildup and hardening of the arteries.  

The Medical News Today’s experts weigh-in on what post-menopausal women can do to protect their health. The top recommendation involves adopting a healthy lifestyle such as increasing exercise, managing stress, maintaining a healthy weight and eating a more plant-based diet. Moderating your alcohol consumption and not smoking are also important.

Maintaining Good Balance as You Age

Falls among senior citizens are all too common, and in many cases, threaten a person’s independence and way of life. Injuries from falls include to the head, hip fractures, and in extreme cases lead to death; and poor balance is one of the reasons people fall.

Experts explain a person’s balance declines with age due to loss of sensory elements, the ability to integrate information and issue motor commands; and loss of musculoskeletal function as well as poor vision and inner ear problems. Certain medications and common medical conditions in older adults can also cause problems. Balance begins to decline in midlife, starting at about age 50. In one study, adults in their 30s and 40s could stand on one foot for a minute or more. By age 50, the time decreased to 45 seconds.

The National Institute on Aging (NIH) explains maintaining good balance as you age and learning about fall prevention can help you get around, stay independent, and carry out daily activities. Targeted exercises like walking, strength training and workouts designed to improve stability are some of the best ways to improve balance. Consult with your doctor as to whether a physical therapist or other professional could help design suitable exercises to improve your balance. 

Also talk to your doctor about any dietary changes you should make if your balance problems are due to high or low blood pressure. Something as simple as eating less salt, maintaining a healthy weight and exercising may help with high blood pressure. 

If you have low blood pressure and experience dizziness, do not stand up too quickly, drink plenty of fluids such as water and avoid alcohol. Your doctor may need to evaluate if it is safe for you to drive. To reduce your risk of falling, avoid walking in the dark, wear low-heeled, rubber-soled shoes; and if you need a mobility aid such as a cane or walker, use it.

Bone Loss is Common in Older Adults

The Mayo Clinic explains our bodies constantly absorb and replace bone tissue. When a person has osteoporosis, their body is removing old bone but not keeping up with making new bone. Early warning signs of osteoporosis include bone fractures, lost height, curved back, sudden back pain and gastrointestinal problems. However, many people have no symptoms, and it isn’t until a bone fracture or a bone density test (measuring the proportion of minerals in the bones), do they learn they have osteoporosis. A step above osteoporosis is osteopenia, which affects approximately half of Americans over age 50.  

It is important to work with your doctor if your bone loss has progressed to osteoporosis as there are medications that can help build stronger bones. In addition, you can help increase bone strength with a healthy diet of foods high in calcium, vitamins D and K, and protein. Foods containing calcium include dairy and soy, fish and green leafy vegetables like kale. Additionally, many foods are fortified with calcium so check labels. Protein is found in meat, fish, eggs, soy and dairy products, beans, lentils, and legumes as well as seeds and nuts. Experts recommend people over age 70 increase their uptake of vitamin D to 800 IU daily, which they can also obtain from supplements. 

Consuming a lot of salt should be avoided as the National Institute of Arthritis and Musculoskeletal and Skin Diseases reports overconsumption of salt can cause the kidneys to excrete calcium. Studies indicate consuming caffeine in coffee or caffeinated drinks can additionally interfere with the body’ absorption of calcium and it is recommended older adults limit their coffee intake to no more than three cups a day. Limiting alcohol also helps maintain bone health. Another component to maintaining or building stronger bones includes weight-bearing exercises.

Muscle Loss in Older Adults

The term sarcopenia refers to age-related muscle loss and is a natural part of growing older. However, it also may occur at a faster pace after prolonged periods of inactivity from injury, illness or the pain of osteoarthritis. Sarcopenia causes muscle atrophy and brings on problems such as poor balance and greater weakness, which in turn, can result in more inactivity and increased risks for falls.   

Seniors with muscle atrophy may have difficulty reaching their arms upward, opening jars, typing on a keyboard or even tying shoes.  The Journal of Rehabilitation Medicine reports on a study that found “older men who did eight weeks of strength training lost about 25% of their muscle gains after they stopped training for two weeks.”

Jodi Klein, a physical therapist with Harvard’s Brigham and Women’s Hospital, suggests a few things you can do if you know you have a surgical procedure ahead of you that could cause muscle atrophy. She says if you are currently active, keep it up; if not, use this time to get started.  Klein explains, “The better shape you are in going into surgery, the better shape you will be coming out, and the less chance you have of enduring muscle loss.” 

Older adults who want to regain muscle strength should talk to their health care provider about the best methods. Experts recommend an exercise routine that builds both upper and lower body strength. This could include weight training with dumbbells and resistance bands in conjunction with walking, swimming, and biking. If it would be helpful, think about turning to an expert to aid you in regaining strength.

As we age, it becomes more important than ever to protect our health but making decisions on how we do it can be confusing at best. One day it seems this diet is better than another and they have lots of initials such as DASH, MIND, TLC or KETO as well as Mediterranean, vegan, vegetarian and flexitarian diets. Adding to the challenge is whether we need a daily multivitamin. 

While it is a good idea to get advice from your healthcare provider, it doesn’t hurt to hear what some other experts say. For example, the Harvard School of Public Health says if you eat a healthy diet, a multivitamin may have little or no benefit. A healthy diet should include plenty of vegetables, fruits, whole grains, good protein sources and healthy fats. 

However, the National Academy of Medicine cautions some Americans may not be getting enough of specific vitamins and minerals such as vitamin D and vitamin E from food sources alone. Vitamin deficiency can also occur in people with diseases like celiac, ulcerative colitis, or cystic fibrosis, which cause their digestive systems to not function properly. Others who may benefit from the addition of specific vitamins include the elderly or conditions such as pregnancy, alcoholism and taking certain medications including treating high blood pressure. 

Doctors say even those with diseases such as osteoporosis may only need to increase their intake of nutrients targeted toward maintaining bone health, or if that is not possible to take a supplement. If in doubt, ask your healthcare team if you should consult with a dietician. Remember to let your doctors know about any supplements you are taking in case of drug interactions with prescription medications.

Cataracts Are the Most Common Age-Related Eye Condition

The National Institute of Health (NIH) reports if you are lucky enough to live a long life, along the way, you will develop cataracts. While it may not get to the point of requiring surgery, they say everyone eventually has them. However, while cataracts are the most common age-related eye condition, they are also the leading cause of preventable blindness.

So how do you know if you have cataracts or if it is time for surgery? Ophthalmologists explain the most accurate way to keep track of your eye health is to get regular eye exams; but there are some signs you can look out for on your own. One of the top changes you may notice is more difficulty driving at night. Cataracts make it much harder to see in the dark. Reading may also become difficult as you find yourself needing brighter and brighter light to focus on the page. 

Other symptoms can include seeing everything through a slightly tinted yellow or brown hue; or experiencing halos around light sources such as headlights or streetlights. Some cataracts can cause double vision, and between seeing halos and double vision makes night driving especially dangerous. Experts say if you are experiencing any of these symptoms, please make an appointment to have your eyes checked.  

If surgery is recommended, remember every year more than four million cataract surgeries are performed in the United States. It is important to follow your doctor’s advice for a speedy recovery. Your eyes will be frail for several weeks afterward but most people have little to no pain and experience good vision almost immediately.

Food to Enjoy without the Guilt

When dieting, it is best to shy away from fried foods, foods loaded with empty carbs and sugary foods. Dieticians say that still leaves a lot of tasty food we can eat without feeling guilty. At the top of the list are non-starchy vegetables of which there are many ways to prepare them to ensure they are both satisfying and tasty. 

If vegetables aren’t your favorite foods, it may be you are not creative enough in preparing them. A simple suggestion is to give your veggies a quick spray of olive oil, add seasoning and a few cloves of garlic. Either roast or air fry to give them a punch of flavor. If you love dips, think about switching up from heavy-caloric dips to ones using salsa or low-fat yogurt. 

A list of veggies nutrient-dense and light in calories includes broccoli, cauliflower, spinach, carrots, sprouts and kale. If you want fruits, the best to eat are watermelon and cantaloupe, most berries and kiwi. Beata Rydyger, a registered nutritionist based in Los Angeles, says “Along with these foods being beneficial for weight maintenance and weight loss, they are very high in macronutrients such as carbohydrates and micronutrients including vitamins and minerals.” Moreover these foods are rich in fiber, and fiber keeps blood sugar levels stable and keeps you feeling fuller longer.    

Other foods that fill you up are nuts and seeds. While they are higher in calories than fruits and veggies, a handful contains protein as well as vitamins and minerals. Snacking mid-afternoon on a cup of low-fat cottage cheese, hard-boiled egg or cup of tuna helps ensure you won’t overeat at dinner time. Remember to stay hydrated throughout the day. The Harvard Health Publishing journal recently reported on a study of a number of studies focused on the effectiveness of music therapy. The study found listening to music, singing and other forms of music therapy “can create significant improvements in mental health and smaller improvements in physical health–related quality of life.” Researchers sum it up by saying music offers a positive impact on the psychological quality of life; and is an easy way to raise spirits and help us feel better.   

So if we feel music makes us happier and more relaxed, it is not our imagination. AARP quotes Laurie Keough, a clinical associate professor of music therapy at Nazareth College in Rochester, New York, who explains, “Music is processed all throughout the brain;” which is why it is applicable in so many different situations. 

Music therapy is an established form of therapy with a lot of evidence of the benefits. These include improving health care goals like decreasing anxiety, improving mood and decreasing pain perception among others. Moreover since listening to music is one of the best ways to get the benefits; it is easy for all of us to do.

Researchers say as we listen to music, it triggers the release of dopamine, a neurotransmitter that’s part of the brain’s reward system, making us feel happier. It can also distract from negative thoughts and help us focus on more pleasurable emotions. A soothing playlist can be used to quiet the mind to help transition to sleep. Depression is Not a Normal Part of Growing Older

Depression can affect people at any age including senior citizens.  According to Web MD, “Late-life depression affects about 6 million Americans age 65 and older, but only 10% receive treatment for depression.”  They suggest the reason for this may be that the symptoms seniors display are often ignored or assumed to be a result of illnesses and medications or just a normal part of growing older. 

There are many reasons why seniors suffer from depression including major changes in their lives such as retirement, the death of a spouse, illnesses, disabilities and social isolation.  People suffering from very painful, disabling, or life-threatening illnesses are especially vulnerable to depression or the worsening of symptoms.  Furthermore, depression can be caused by medications that are commonly prescribed to the elderly including blood pressure medication, beta-blockers, sleeping pills, tranquilizers, calcium-channel blockers and medication for Parkinson’s as well as others.

Suffering from depression prevents seniors from enjoying life as they used to and can be the cause of a change in appetite, problems sleeping and impact physical health.  HelpGuide.org says, while sadness is one sign of depression, some seniors do not recognize or admit to feelings of sadness.  Rather someone may “complain instead of low motivation, a lack of energy, or physical problems.  In fact, physical complaints, such as arthritis pain or worsening headaches, are often the predominant symptom of depression in the elderly.”

All too often seniors just accept depression as a part of the aging process but there are steps you can take to help yourself or your loved one.  You first need to talk to your health care provider to rule out other problems, and then perhaps you need to make changes in your lifestyle.  Experts suggest that isolation and inactivity can make depression worse and they recommend getting involved in physical, mental and social activities.

Maintaining Good Balance as You Age

Falls among senior citizens are all too common, and in many cases, threaten a person’s independence and way of life. Injuries from falls include to the head, hip fractures, and in extreme cases lead to death; and poor balance is one of the reasons people fall.

Experts explain a person’s balance declines with age due to loss of sensory elements, the ability to integrate information and issue motor commands; and loss of musculoskeletal function as well as poor vision and inner ear problems. Certain medications and common medical conditions in older adults can also cause problems. Balance begins to decline in midlife, starting at about age 50. In one study, adults in their 30s and 40s could stand on one foot for a minute or more. By age 50, the time decreased to 45 seconds.

The National Institute on Aging (NIH) explains maintaining good balance as you age and learning about fall prevention can help you get around, stay independent, and carry out daily activities. Targeted exercises like walking, strength training and workouts designed to improve stability are some of the best ways to improve balance. Consult with your doctor as to whether a physical therapist or other professional could help design suitable exercises to improve your balance. 

Also talk to your doctor about any dietary changes you should make if your balance problems are due to high or low blood pressure. Something as simple as eating less salt, maintaining a healthy weight and exercising may help with high blood pressure. 

If you have low blood pressure and experience dizziness, do not stand up too quickly, drink plenty of fluids such as water and avoid alcohol. Your doctor may need to evaluate if it is safe for you to drive. To reduce your risk of a fall, avoid walking in the dark, wear low-heeled, rubber-soled shoes; and if you need a mobility aid such as a cane or walker, use it.

A Person’s Gait may Signal Early Signs of Alzheimer’s

Several new studies including one from the journal Current Biology, report the way a senior walks may be messaging something about their future cognitive health. Researchers say walking relies on a person’s navigational skills, which demands mental engagement. 

A slowing of a person’s walking pace is one of the things studies are looking into as an early warning sign of Alzheimer’s. People may start to take smaller steps and drift from side to side as well as exhibit poor balance and a history of frequent falls.  Ryan Glatt, a certified personal trainer, senior brain health coach, and director of the FitBrain Program at Pacific Neuroscience Institute in California, told Medical News Today, “In clinical settings, gait-based tests can be helpful for determining fall risk, especially with distractions, and perhaps can be part of a more comprehensive cognitive evaluation in the near future.”

Other studies report people with early Alzheimer’s have difficulty turning when walking. Researchers say the problem was not detected in older, healthy participants who suffered from mild cognitive impairment, suggesting the problem is specific to Alzheimer’s. Glatt explained the reason might be connected to changes in the hippocampus, a region of the brain partially responsible for spatial memory that helps to control visuospatial navigation.

Another early warning sign of Alzheimer’s is an altered sense of a person’s environmental perceptions. For example, a person may struggle to understand objects, and become confused by thinking something is 2D when it is really 3D; or vice versa.  

Dr. Jonathan Fellows, who leads the Michigan Institute for Neurological Disorders, Alzheimer’s Disease and Memory Disorder Center, adds, “Observation of gait is an important assessment in the diagnosis of various dementias, including Alzheimer disease, as patients with this disorder can have what is called dyspraxia, or difficulty doing a previously learned task – in this case walking or turning.” Researchers explain some of the problems that appear to be specific to Alzheimer’s might allow doctors to diagnose patients earlier; and without the need for blood, spinal fluid or speech tests.

The Role of Probiotics and Prebiotics in Gut Health

The last several years have brought probiotics and prebiotics into the spotlight. However, it can be confusing as to what the difference is between the two and the role they play in our bodies. Experts say both are important to our health; probiotics are beneficial bacteria, while prebiotics are food for these bacteria.

In other words, “Probiotics are live bacteria found in certain foods or supplements and can provide numerous health benefits. Prebiotics are substances from types of carbs (mostly fiber) that humans can’t digest. The beneficial bacteria in your gut eat this fiber.”

The Harvard Medical School explains our large intestine has 100 trillion good bacteria essential to our health called microbiome. These good microbes help maintain healthy bowel function. Each person has their own unique microbiome from birth and we add to the good microbes by the food we consume. However, a diet high in sugar and fat can negatively affect gut health by encouraging the growth of bad bacteria, allowing them to grow faster and colonize more easily, without as many helpful bacteria to prevent it from happening. 

Research shows certain foods like yogurt and kefirs with live, active cultures are beneficial to adding to our microbiome. Unpasteurized, fermented foods like kombucha, miso, sauerkraut and sour pickles contain healthy microbes but most packaged fermented foods are pasteurized, which destroys the microbes. The best way to get around that is to learn to ferment foods at home.

Experts say while there are many probiotic supplements on the market, it is not easy to find exactly what you need. Most probiotic supplements contain just a few bacterial strains, compared to the many stains naturally found in the gut. “Although there are many claims made by probiotic products, there’s now reliable, evidence-based support that certain probiotics — like Lactobacillus, Bifidobacterium (bacteria), and Saccharomyces boulardii (yeast) — are generally safe and helpful in specific conditions.”
