The Basics of Medicare

If you recently had a medical procedure or were hospitalized, you may be confused about a recent bill and are wondering what Medicare will cover, what your gap insurance will cover, and what you will be responsible for. It may help to have a better understanding of the four main parts of Medicare.

Part A helps pay for inpatient stays in hospitals and skilled nursing facilities, some home care and end-of-life hospice care that accept Medicare. Hospital services covered include a semi-private room and meals, general nursing, drugs and supplies, inpatient rehabilitation, and some blood transfusions. Skilled nursing services include care for things like recovering from hip replacement surgery or stroke. Part A charges a deductible for hospital stays (the fee for 2025 is $1,676) but your Medigap insurance may cover that expense as well as other deductibles. Hospital stays for up to 60 days and virtually most services are covered by Medicare; but the one exception is not in a private room.  

Part B covers doctors and outpatient services, and together with part A make up what is considered “Original Medicare.” Part B pays for diagnostic screenings, lab tests, preventive services, some medical equipment and transportation. It covers doctor visits and other outpatient services. There is a monthly premium with Part B, and in 2025 it is $185. For those making over $106,000 annually it may be higher. 

Part C is what Medicare Advantage comes under, and is privately held insurance rather than Medicare, which is run by the federal government. If you choose Medicare Advantage, you will still need to sign up for Medicare part A and B and pay the premium for part B. Medicare Advantage plans set their own deductibles and you typically need to stay in a Network and use your plan’s approved providers or pay higher out of pocket costs. 

Part D is again from a private insurer and pays for some of your prescription drugs. Generally there is a co-pay for each prescription, and you may have a premium, and a yearly deductible. Check with Medicare regarding rules for higher yearly drug costs and your responsibility.

Remember Medicare offers an open enrollment period each year to allow recipients to make changes to their plan. The period starts on October 15 and ends December 7. Any changes you make during this time will go into effect the following year.

Myths About Over-the-Counter Medication

Medical professionals worry far too many people do not use over-the-counter (OTC) medications according to stated directions. The most commonly used OTC medications are for pain management, allergy treatments and to reduce the symptoms of flu. 

US News & World Report conducted a survey in March 2023 of more than 350 pharmacists nationwide from chain, independent and hospital pharmacies regarding their views on patients’ compliance with OTC drugs. The results showed 9 out of 10 pharmacists agreed consumers believe too many myths about OTC medications.

The list of some of the most commonly-held misconceptions includes:

  • It’s okay for children to take adult OTC medications as long as the doses are reduced.

  • It’s okay to double-dose OTC medications when I forget a dose.

  • Natural or organic supplements are safer than other OTC medications.

  • The bathroom is a good place to store OTC medications.

  • It’s okay to take OTC medications after their expiration dates.

  • It’s okay to cut, crush or chew OTC pills.

  • Brand name OTC medications or products are clearly better than their generic versions.

Debunking the first item, experts explain it is not okay to give infants and young children adult medications as they need smaller doses that are best measured using appropriately designated formulations. They remind the public there are many medications designed specifically for children. Hanna Phan, clinical associate professor at the University of Michigan College of Pharmacy in Ann Arbor, Michigan, says, “Substituting adult forms, which are often tablets or capsules, can be inaccurate and may increase risk for medication errors, such as overdose.” 

Other myth busting information in assisted living is to store OTC drugs in a dry, cool spot as recommended for prescription medications. Pharmacists do say it is probably okay to take an OTC a few months after the expiration date; but much longer and the active ingredients may have lost their effectiveness. Also, they say for the most part generic versions of a product are as good as a brand name one. Another warning is to not double up on a medication even when you miss a dose. Moreover, just because something is labeled as organic doesn’t mean it is always safe for you to take. Rather check with your doctor in assisted living.

The Alzheimer’s Association is a great resource to become familiar with and offers topics ranging from Alzheimer’s symptoms to caregiving and medical breakthroughs. Experts say it is important to remember you are not alone. Over six million people in the US are living with Alzheimer’s and millions more know someone with the disease. Alzheimer’s five distinct stages include: Mild cognitive impairment (MCI); Mild; Moderate; Severe; and Very Severe. Symptoms can vary from person to person but the various stages of Alzheimer’s disease have some common characteristics and it helps to be prepared. 

To help negate the feeling of being a complete victim of your circumstances, studies show there are lifestyle changes you can make in the early stages to help improve brain health. Treating Hearing Loss May Reduce the Risk of Developing Dementia

JAMA Neurology added to the growing body of evidence showing a link between hearing loss and cognitive impairment. The study found treating hearing loss with hearing aids may reduce the risk of developing dementia by up to 19%. In fact, researchers say hearing loss may be the biggest treatable factor for dementia. An article in AARP explains what may cause the link between hearing loss and dementia. 

The first theory regards socialization. Scientists say once a person starts having more difficulty being in a group of people and following the conversation, they may avoid such social gatherings. This in turn can lead to isolation, which is problematic as less social engagement as well as less cognitively stimulating activities and depression are risk factors for dementia. 

Secondly, coping with hearing loss means the person is constantly working to sort out sounds and make sense of what is being said. The brain now may have “fewer resources (brain power) to help preserve thinking and memory abilities.” Scientists also believe hearing loss causes the brain to be less stimulated with sound information, which is linked with parts of the brain shrinking and atrophying faster with chronic deprivation.  

While experts agree we still need more studies regarding the link between hearing loss and a higher risk for dementia, they do know that addressing a hearing issue with hearing aids is beneficial for the person’s overall quality of life. Moreover, there are no negative medical issues associated with the devices.

Some Lifestyle Habits Can Increase Your Risk of Developing Arthritis

Many people assume arthritis comes with aging, but experts say while joint pain is common in older adults, arthritis is not a normal part of aging. The CDC explains arthritis is a group of more than 100 conditions causing inflammation and swelling in the joints or nearby tissues with symptoms such as pain and stiffness. Osteoarthritis is the most common type of arthritis and mostly occurs in older adults (about 70% are 55 years and older). Other common types of arthritis are rheumatoid arthritis and gout. 

While there is not a cure for arthritis, there are treatments, medications and therapies. In addition, studies find healthy lifestyle habits can help manage symptoms of osteoarthritis, rheumatoid arthritis, and gout as well as lessen your risk for even getting it. For example, the CDC reports 40% of people with arthritis can decrease pain and improve function by being physically active. 

Osteoarthritis is the breakdown of the cartilage of the joints causing bones to rub against one another resulting in pain and difficulty moving the joint.  Experts say millions of cases could be prevented by avoiding excessive weight gain. Exercising regularly and doing resistance training is another way to avoid injury to the joints. 

Rheumatoid arthritis (RA) is a chronic condition causing inflammation and swelling in joints. It can misshape, misalign and damage joints; and if it is in one knee or hand, it is typically on the other hand or knee. Recommendations to reduce your risk for RA include avoiding tobacco. Gingivitis, or gum inflammation, is linked to several health conditions including RA; and prevention includes improving oral health with good hygiene and dental visits.  

Gout is another common form of arthritis and occurs when crystals of uric acid are deposited in one or more joints. People with gout experience pain, swelling and redness in the affected joints. Experts say eating a healthy diet, limiting alcohol, and staying well hydrated are good steps to reduce your risk. If you are overweight, lose weight and if possible avoid diuretics (discuss all medication changes with your doctor). Studies show regardless of what type of arthritis you have, early diagnosis and treatment can make a big difference in your future joint health.

By organizing an activity such as scrapbooking, it can help to keep everyone engaged as well as inspire your loved one to start sharing stories about the “good old days.”  As you start documenting their life story, it will likely bring back memories they may have forgotten and/or the rest of the family has never heard.  Along with exercising the brain with recollections of the past, scrapbooking can boost self-esteem, improve one’s dexterity and increase socialization by introducing new topics of conversation.

Considerations For Older Drivers

Older drivers can still be good, safe drivers, but there are some things to consider both for your safety and the safety of others.  The Mayo Clinic recommends that seniors stay as active as possible in their day-to-day life which helps to maintain overall strength, flexibility, and range of motion.  Drivers need to be able to turn and look over their shoulder and move their head with ease.  They must be able to keep a firm grip on the steering wheel and legs need to be strong enough to apply pressure to the gas pedal or brake.  

A senior’s peripheral vision can decrease with age or from certain medications and other eye problems may cause drivers to have trouble seeing in the dark or experience blurred vision.  Hearing problems can interfere with detecting sounds of sirens from emergency vehicles or approaching cars.  Be vigilant about scheduling vision and hearing tests regularly as part of your wellness care and follow your doctor’s advice as to how often they should be done.  

Remember that certain medications can cause drowsiness; review the side effects of your prescription drugs and do not drive if you are feeling sleepy.  It may be a good idea to limit trips in the car to daylight hours and when the weather is good.  Keep in mind that a person’s reaction time slows with age as do their motor skills.

Planning before you leave:

  • Plan to drive on streets you know.

  • Limit your trips to places that are easy to get to and close to home.

  • Take routes that let you avoid risky spots like ramps and left turns.

  • Add extra time for traveling if driving conditions are bad.

  • Avoid distractions such as listening to the radio or having conversations.

  • Leave plenty of space between your car and the one in front of you. If you are driving at higher speeds or if the weather is bad, leave even more space.

How to Better Connect With Someone With Alzheimer’s Disease

A Yale Medicine neurologist offers advice on how to better connect with someone with Alzheimer’s disease. Carolyn Fredericks, MD., a specialist in cognitive and behavioral conditions, including Alzheimer’s, says it is not always helpful to correct a person with Alzheimer’s, and in most cases, it is nicer to just “enter their reality.” 

Fredericks suggests instead of correcting the person when they get your name or someone else’s name incorrect, just go with it. After all, what difference does it really make? Also, you don’t need to remind the person you answered their question an hour ago. Let it go and repeat the answer.  

Going along with what the person says, and remaining calm and encouraging will help them remain calm. Research indicates someone with Alzheimer’s is typically most anxious about making mistakes or forgetting something they should know in the early stages of the disease. Moreover, when they repeat a question, they are not trying to be difficult, so there is no reason to make them feel uncomfortable. If the person is unduly worried about something that is not true such as thinking things that go “missing” are being stolen, show compassion and focus on helping them find the items.   

As long-term memories are more intact in people with Alzheimer’s and often not affected until the later stages of the disease, spending time reminiscing about things from their childhood can be fun for you and the person. Even in advanced stages of Alzheimer’s, people find joy in listening to their favorite music or participating in creative projects. Be sure they get some exercise each day as it helps with anxiety and sleep. Lastly, Dr. Frederick says it is important for the family to construct a safety plan and recognize when it is no longer safe for the person to drive or use the stove when he/she is alone.

From there you can call to inquire about availability and pricing, and if the place sounds good, go ahead and book an in-person tour. While at the community, make the effort to speak to as many people as possible. Try to be there during an activity to see how engaged residents are, and join the group for lunch if you have the opportunity.
