Living With Mild Cognitive Impairment

Mild cognitive impairment (MCI) can be a precursor to dementia but does not always progress to dementia. Medical professionals report 10% to 40% of MCI cases go on to become dementia, and a good percentage of those become Alzheimer’s disease. On the positive side, research shows as many as 30-40% of people with MCI get better. As with Alzheimer’s disease and other types of dementia, there are no drugs to treat MCI but doctors say there are steps we can take to mitigate or minimize its effects. 

MCI is just as its name suggests. The impairment is mild and does not affect a person’s ability to care for themselves or take away their independence. Steps a person should take to protect their cognitive health include quitting smoking and moderating or eliminating alcohol consumption. If you do have hypertension (high blood pressure), high cholesterol levels or diabetes, it is important to treat these conditions and manage them through medications, diet and healthy lifestyle habits. 

Other things you can do to thwart, delay or minimize the effects of MCI are to maintain a healthy weight and exercise, which is good for our brains as well as our bodies. Avoid junk food, highly processed foods and foods high in sodium; and reduce the amount of red meat in your diet. Isolation has also been linked to a higher risk for dementia, so it is important to stay social.

Help Stop Fraud by Protecting Your Medicare Number

The National Health Care Anti-Fraud Association estimates last year’s annual Medicare insurance fraud was around $100 billion, and most likely much higher. While one in five beneficiaries say they have encountered potential Medicare fraud, there are precautions you can take to reduce or eliminate fraudulent claims starting with protecting your Medicare and Social Security cards and numbers like you do your credit cards. 

Experts say don’t respond to a call, text or email asking you to provide your Medicare Number. They caution you to only show your Medicare card or give out your number to your doctor or people you know who should have it. To determine if you are a victim of fraud, examine your Medicare Summary Notices (MSNs) or claims statements carefully for any discrepancies or additions of services or charges you did not receive. Another common scam is to try to get Medicare beneficiaries to sign up for drug plans that are not approved by Medicare. 

If your Medicare card is stolen or you suspect fraud, report it as soon as possible to 1-800-MEDICARE (1-800-633-4227). It is important to know Medicare will never call to sell you anything or visit you at home. They will only call or contact you in limited situations to ask for personal information. One such person may be the agent who helped you sign up for a particular Medicare program or drug plan. You also may be contacted by Medicare if you left your number to report fraud or inconsistencies with your MSNs.

Extreme Heat is Particularly Hard on Older Adults

Most areas in the United States have seen summers getting hotter over the last several years. Several days in a row of extreme heat can especially take a toll on seniors. Experts say if you are an older adult and/or someone on medications and have chronic heart, lung or kidney problems, it can make it even harder for your body to handle higher temperatures. 

While hotter temperatures are hard on all of us, studies find seniors do not adjust as well to heat and humidity as they did when they were younger. The National Institute on Aging says it is important for those at risk to take precautions and get relief quickly when you do feel overheated. 

Along with a slower response to temperature changes, prescription medication like high blood pressure and heart medications; diuretics and sedatives make it harder for the body to cool. In addition, many seniors may be on several medications that only compound the problem. 

Other issues interfering with seniors ability to handle the heat is obesity or being overweight or underweight as well as normal changes in aging skin. Not drinking enough fluids can lead to dehydration and increase your risk of heat-related illness. Water, fruit juices and drinks with electrolytes are good choices but avoid caffeinated products and alcohol.

If you live in a place without air conditioning, it is important to keep the space as cool as possible. Limit the use of the oven and keep blinds and drapes shut during the hottest part of the day. Open windows at night to bring in cooler air. If possible, spend part of your day in a cool spot such as an indoor mall, cooling center or library where there is air conditioning. If you need transportation, don’t stand outside waiting for a bus, ask a relative or friend for a ride or call a ride service.   

The NIA says signs of heat-related illness include headache, confusion and dizziness. If you experience any of these symptoms, they advise going to the doctor or an emergency room to find out if you need treatment.

Habits That Are Especially Dangerous for Cognitive Health

Our modern-day lifestyle encourages behaviors that are bad for the health of our brain as well as other parts of our body. Research finds the following four habits are especially dangerous for cognitive health. They are sitting, lack of socializing, inadequate sleep, and chronic stress. The good news is they are habits we can easily change. 

Many of our workplaces and entertainment pursuits include sitting for long periods of time. In fact, a 2018 study published in the PLOS One reports people aged 45 to 75 sit for an average of 6 1⁄2 hours a day. Researchers found “sitting too much is linked to changes in a section of the brain essential to memory.” Using MRI scans, they looked at the region of the brain known to make new memories called the medial temporal lobe (MTL). Comparing the number of hours a day the person spent sitting, those who sat the most had the thinnest MTL regions, a precursor to cognitive decline and dementia. To help improve the functioning of your MTL, the study’s authors recommend getting up and moving at least every 30 minutes. Your activity could be a quick round of squats or pushups or a walk around your front yard, or something similar for a few minutes. 

Lack of socialization can also lead to a thinning of the MTL as several studies link loneliness to a higher risk of dementia. Experts say you don’t have to completely change your lifestyle but rather try to routinely connect with three or four family members or friends. The number of social connections is less important than the degree of meaningful interactions.  

Other healthy cognitive behaviors include getting at least seven to eight hours of sleep each night and reducing your level of stress. Research shows chronic stress “can kill brain cells and shrink the prefrontal cortex, the area responsible for memory and learning.” To manage stress, experts suggest breathing techniques and practicing mindfulness.

You may want to plan in advance an activity your loved one would enjoy doing with you during your visit.  Examples include bringing along a puzzle to work on, a Scrabble board or a trivia game.  You could get prints of current photos of the family made to add to an album or start a new one.  If they are a music lover, bring some new music to enjoy together.  If they enjoy listening to stories, get an audiobook to listen to or buy a favorite family movie to watch together.  If they like sending greeting cards to family and friends, buy a box of cards and help get them ready to mail.   If they get pleasure from being pampered, bring along the supplies to give them a manicure.

Older Adults Are at Greater Risk for Serious Eye Diseases

Just as the rest of our body ages, so do our eyes. Older adults often complain of dry, teary or tired eyes and may have problems driving at night, experience flashes and floaters or need glasses to read. As we grow older, we are also at a higher risk for serious eye diseases that can impact our vision. 

One of the most serious eye diseases is age-related macular degeneration (AMD). The American Macular Degeneration Foundation reports AMD is the leading cause of irreversible vision loss among people aged 60 and older, affecting nearly 20 million Americans. The risk of developing AMD increases with age and if you are 75 or older, your risk is 1 in 3.  Age-related macular degeneration causes the macula, a part of the back of the eye near the retina, to deteriorate. Experts say while it doesn’t impact peripheral vision, macular degeneration can affect how well a person sees what’s right in front of them.

You are at a higher risk of getting AMD if a relative has had it or you are Caucasian, and females are more susceptible to AMD than males. Lifestyle choices also increase your risk for AMD including smoking along with elevated blood pressure and cholesterol levels. Sun exposure is another risk factor and doctors recommend wearing sunglasses and a wide-brimmed hat. Maintaining a healthy diet by eating foods high in antioxidants like lutein found in green, leafy vegetables such as spinach are beneficial for our eye health.

Symptoms of AMD include visual distortions and can make straight lines appear bent, reduce central vision in one or both eyes, increase blurriness of printed words, and the need for brighter light when reading or doing close-up work. The first signs of AMD are typically discovered by an eye doctor during an annual dilated eye exam.

In fact, the Agency for Healthcare Research and Quality (AHRQ), a division of the U.S. Dept. of Human Health & Services, reminds all of us about the benefit of preparing questions and accurate information ahead of time to provide to your physician. They say it not only improves your care, but builds trust between you and your healthcare provider, which can lead to better results.  

For help on questions to ask to facilitate a more fruitful visit to your doctor and efficiently use your precious time (studies show doctors spend between 13 to 24 minutes with each patient), the AHRQ offers a series of questionnaires related to your visit. Remember typically you have a reason for the visit such as: a well visit, looking for answers for a specific medical problem, returning to discuss treatment, or getting test results. Prepare for your appointment by bringing your prescription medications as well as over-the-counter drugs you use including vitamins and dietary or herbal supplements. 

When discussing a possible medical procedure, AHRQ says questions should include: why do I need this procedure; how do I prepare; when will I get the results; and how will I feel afterward? If surgery is recommended, ask if there are alternative treatments available that are just as effective; possible complications; and recovery period. AHRQ says if you are given a new medication, ask how the new drug will interact with other drugs you are taking, how long you will be on it, and common side effects. Before you leave your appointment, make sure you understand your instructions.
